血液灌流联合透析对慢性肾病患者残余肾功能及血清VEGF、ET⁃1的影响

摘　　要：目的探讨血液灌流联合血液透析治疗法对慢性肾脏疾病患者残余肾功能、血管内皮细胞生长因子(VEGF)及血管内皮素⁃1(ET⁃1)水平的影响。方法选取河南省平顶山市第一人民医院2019年12月至2020年12月收治的108例慢性肾脏疾病患者,按照随机数字表分为实验组和对照组,每组各54例,对照组采用常规血液透析,实验组采用血液灌流联合血液透析治疗,比较两组患者治疗后的临床疗效、残余肾功能、VEGF、ET⁃1、肿瘤坏死因子⁃α(TNF⁃α)、白细胞介素(IL⁃6)、C⁃反应蛋白(CRP)、白细胞介素(IL⁃10)。结果实验组有效率为92.59%,明显高于对照组的77.78%,差异有统计学意义(P<0.05)。治疗3个月及半年后,两组残余肾功能均降低,但实验组高于对照组,差异有统计学意义(P<0.05)。治疗后,两组VEGF、ET⁃1、TNF⁃α、IL⁃6、CRP、IL⁃10水平均降低,且实验组低于对照组,差异有统计学意义(P<0.05)。结论血液灌流联合血液透析治疗对慢性肾脏疾病患者的残余肾功能有一定的保护作用,能清除中、大炎性分子物质,降低血清VEGF及ET⁃1水平,提高临床疗效。Objective To explore the effect of hemoperfusion combined with hemodialysis on residual renal function and levels of vascular endothelial growth factor(VEGF)and vascular endothelin⁃1(ET⁃1)in patients with chronic renal disease.Methods 108 patients with chronic renal disease admitted to Pingdingshan First People’s Hospital,Henan Province were selected from December 2019 to December 2020,and they were divided into the experimental group and the control group according to the random number table method,with 54 cases in each group.The control group was given conventional hemodialysis and the experimental group was given hemoperfusion combined with hemodialysis.The clinical efficacy,residual renal function and levels of VEGF,ET⁃1,tumor necrosis factor⁃α(TNF⁃α),interleukin(IL)⁃6,C⁃reactive protein(CRP)and interleukin(IL)⁃10 after treatment were compared between the two groups.Results The effective rate of the experimental group was 92.59%,which was significantly higher than that of the control group,77.78%,and the difference was statistically significant(P<0.05).After 3 months and half a year of treatment,the residual renal function in both groups decreased,but the experimental group was higher than the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of VEGF,ET⁃1,TNF⁃α,IL⁃6,CRP,and IL⁃10 in the two groups were decreased,and the experimental group was lower than the control group,and the difference was statistically significant(P<0.05).Conclusion Hemoperfusion combined with hemodialysis has a certain protective effect on the residual renal function of patients with chronic renal disease.It can remove medium and large inflammatory molecules,reduce the levels of serum VEGF and ET⁃1,and improve clinical efficacy.
