豫西某三甲医院1521例皮肤病患者食物不耐受特异性IgG抗体检测结果分析

摘　　要：目的分析豫西地区皮肤病患者食物不耐受特异性IgG抗体检测结果,以期为皮肤病患者的食物不耐受及诊疗提供参考依据。方法采用回顾分析研究方法,选择2019—2020年豫西某三甲医院皮肤科患者1521例为研究对象,收集患者一般资料,探讨食物不耐受患者年龄、皮肤病种类与食物不耐受的关系。结果1521例患者中食物不耐受IgG抗体阳性率为70.48%(1072/1521),其中阳性率最高的5种食物为鸡蛋(50.03%)、牛奶(37.34%)、小麦(26.17%)、西红柿(16.44%)、大豆(12.10%)。未成年组对鸡蛋、牛奶、小麦、西红柿、大豆等食物的不耐受发生率明显高于中年组和老年组(P<0.05)。银屑病、荨麻疹、湿疹、过敏性紫癜这4种常见皮肤病患者均对一种或多种食物不耐受,且多重不耐受发生率较高。结论豫西地区皮肤病患者中普遍存在食物不耐受,患者年龄、皮肤病种类与不耐受的食物有一定关系,对血清中特异性IgG抗体进行检测可及时发现不耐受食物,通过调整饮食可避免机体发生免疫反应,减少皮肤病的发生。Objective To analyze the detection results of food intolerance specific IgG antibodies in patients with dermatosis in the western Henan,and to provide reference for the diagnosis and treatment of food intolerance in patients with dermatosis.Methods A retrospective analysis was conducted to investigate the relationship between age,dermatosis types and food intolerance in 1521 patients with dermatosis from a third class A hospital in the western Henan from 2019 to 2020.Results The positive rate of food intolerance IgG antibody in 1521 patients was 70.48%(1072/1521),and the five foods with the highest positive rate were eggs(50.03%),milk(37.34%),wheat(26.17%),tomatoes(16.44%)and soybeans(12.10%).The incidence of intolerance to eggs,milk,wheat,tomato and soybeans in the juvenile group was significantly higher than that in the middle-aged group and the elderly group(P<0.05).Patients with psoriasis,urticaria,eczema and henoch purpura were all intolerant to one or more foods,and the incidence of multiple intolerances was high.Conclusion Food intolerance is common in patients with dermatosis in the western Henan.The age and types of dermatosis are related to the food intolerance.The detection of specific IgG antibody in serum could timely find out the food intolerance,adjust the diet,avoid immune reaction and reduce the occurrence of dermatosis.
