金黄膏外敷对股骨转子间骨折患者术后肿胀程度及髋关节功能恢复的影响

摘　　要：目的:探讨股骨转子间骨折(IFF)术后患者应用金黄膏外敷的效果。方法:纳入2018年7月-2021年7月于南昌市洪都中医院就诊的90例IFF患者,采用随机数字表法将其分为对照组与观察组,每组45例。对照组术后予以常规镇痛、抗感染处理,并积极进行功能锻炼,观察组在对照组基础上加用金黄膏外敷治疗,两组均连续治疗7 d,并随访至治疗后3个月。比较两组临床疗效、肿胀程度、疼痛程度、髋关节功能及血液流变学指标。结果:观察组临床总有效率高于对照组(P<0.05)。治疗7 d后,两组患肢与健肢周径差、VAS评分、全血低切黏度、全血高切黏度、血浆黏度均低于治疗前,且观察组均低于对照组,差异均有统计学意义(P<0.05);治疗后3个月,两组Harris评分均高于治疗前,且观察组高于对照组,差异均有统计学意义(P<0.05)。两组治疗期间均未见明显不良反应。结论:IFF术后患者应用金黄膏外敷治疗,利于减轻患肢肿痛,促进血液循环及髋关节功能恢复,是一种安全、可靠的治疗方案。Objective:To explore the effect of external application of Jinhuang Ointment in patients with intertrochanteric fracture of femur(IFF) after surgery.Method:A total of 90 patients with IFF who treated in Nanchang Hongdu Hospital of Traditional Chinese Medicine from July 2018 to July 2021 were enrolled.They were divided into the control group and the observation group by random number table method,45 cases in each group.The control group was given routine analgesia and anti-infection treatment after surgery,and actively performed functional exercises.the observation group was given external application of Jinhuang Ointment,two groups were treated continuously for 7 d and followed up to 3 months after treatment.The clinical efficacy,swelling degree,pain degree,hip joint function and hemorheology were evaluated in two groups were compared.Result:The clinical total effective rate in the observation group was higher than that in the control group(P<0.05).After 7 d of treatment,the circumferential diameter difference between affected and healthy limbs,VAS score,whole blood low shear viscosity,whole blood high shear viscosity and plasma viscosity in two groups were lower than those before treatment,and the observation group were lower than those of the control group,the differences were statistically significant(P<0.05).At 3 months after treatment,the Harris scores of two groups were higher than those before treatment,and the observation group was higher than the control group,the differences were statistically significant(P<0.05).There were no obvious adverse reactions in two groups during treatment.Conclusion:External application of Jinhuang Ointment for patients after IFF is beneficial to relieve swelling and pain of the affected limb,promote blood circulation and recovery of hip joint function,and is a safe and reliable treatment plan.
