退行性瓣膜病变患者瓣膜置换术后低心排综合征发生的相关因素分析

摘　　要：目的:分析退行性瓣膜病变患者瓣膜置换术后发生低心排综合征的相关因素。方法:选取佳木斯市中心医院二部2020年5月-2021年5月接受瓣膜置换术治疗的退行性瓣膜病变患者97例作为研究对象,术后均进行6个月随访,设计基线资料填写表,记录患者基线资料并记录研究所需资料,统计退行性瓣膜病变患者瓣膜置换术后低心排综合征发生情况,并采用logistic回归分析退行性瓣膜病变患者瓣膜置换术后低心排综合征发生的相关因素。结果:随访6个月结果显示,97例退行性瓣膜病变患者瓣膜置换术后共有22例发生低心排综合征,发生率为22.68%。发生组失血量比、体外循环时间、电解质紊乱及NYHA分级与未发生组比较,差异均有统计学意义(P<0.05)。失血量比>20%、体外循环时间长、电解质紊乱及NYHA分级>Ⅲ级均为退行性瓣膜病变患者瓣膜置换术后发生低心排综合征的相关因素(OR>1,P<0.05)。结论:失血量比>20%、体外循环时间长、电解质紊乱以及NYHA分级>Ⅲ级均为退行性瓣膜病变患者瓣膜置换术后发生低心排综合征的相关因素。Objective:To explore the related factors of low cardiac output syndrome after valve replacement in patients with degenerative valve disease.Method:A total of 97 patients with degenerative valve disease who underwent valve replacement in the Second Department of Jiamusi Central Hospital from May 2020 to May 2021were selected as the research subjects.All patients were followed up for 6 months after operation.Designed the baseline data filling form,recorded the baseline data of patients and recorded the data required for the study,counted the incidence of low cardiac output syndrome after valve replacement in patients with degenerative valve disease,and analyzed the relevant factors of low cardiac output syndrome after valve replacement in patients with degenerative valve disease by logistic regression.Result:The results of 6 months of follow-up showed that 22 cases of 97 patients with degenerative valve disease developed low cardiac output syndrome after valve replacement,with an incidence of 22.68%.There were statistically significant differences in blood loss ratio,cardiopulmonary bypass time,electrolyte disorder and NYHA grade between the occurrence group and the non-occurrence group(P<0.05).Blood loss ratio >20%,long cardiopulmonary bypass time,electrolyte disorder and NYHA grade >Ⅲ were the related factors of low cardiac output syndrome after valve replacement in patients with degenerative valve disease(OR>1,P<0.05).Conclusion:Blood loss ratio >20%,long cardiopulmonary bypass time,electrolyte disorder and NYHA grade >grade Ⅲ are the related factors of low cardiac output syndrome in patients with degenerative valve disease after valve replacement.
