强化降糖治疗联合牙周基础治疗对2型糖尿病伴慢性牙周炎患者血糖控制及牙周健康的影响

摘　　要：目的:探讨强化降糖联合牙周基础治疗对2型糖尿病伴慢性牙周炎患者的血糖控制及牙周健康的影响。方法:回顾性选取2020年2月-2021年2月景德镇市第一人民医院2型糖尿病伴慢性牙周炎患者60例,依据治疗方法将其分为强化组与常规组,各30例。强化组采用强化降糖治疗联合牙周基础治疗,常规组采用常规降糖治疗联合牙周基础治疗。统计分析两组患者的发病间隔、血糖恢复正常时间、体重指数、血糖水平、血脂水平、牙周情况、血清炎症因子水平及满意度。结果:治疗后,强化组患者的发病间隔长于常规组,血糖恢复正常时间短于常规组(P<0.05)。治疗后,强化组的空腹血糖(FPG)、糖化血红蛋白(HbA1c)、总胆固醇(TC)、甘油三酯(TG)均低于常规组(P<0.05)。治疗后,强化组牙石指数(CI)、菌斑指数(PLI)、出血指数(BI)、探针出血(BOP)、探诊深度(PD)、临床附着丧失(CAL)及白介素-1β(IL-1β)水平均低于常规组(P<0.05)。治疗后,强化组血清超敏-C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)水平均低于常规组(P<0.05)。强化组患者的满意度93.33%高于常规组的73.33%(P<0.05)。结论:在2型糖尿病伴慢性牙周炎患者治疗中,强化降糖治疗联合牙周基础治疗较常规降糖治疗联合牙周基础治疗更能有效控制患者血糖,促进患者牙周健康。Objective:To investigate the effects of intensive hypoglycemic therapy combined with periodontal basic therapy on blood glucose control and periodontal health in patients with type 2 diabetes mellitus complicated with chronic periodontitis.Method:A total of 60 patients with type 2 diabetes mellitus complicated with chronic periodontitis in Jingdezhen First People’s Hospital from February 2020 to February 2021 were retrospectively selected.According to treatment methods,they were divided into intensive group and routine group,with 30 cases in each group.The intensive group was given intensive hypoglycemic treatment combined with periodontal basic treatment,the routine group was given conventional hypoglycemic treatment combined with periodontal basic treatment group.The incidence interval,time for blood sugar to return to normal,body mass index,blood glucose level,blood lipid level,periodontal condition,serum inflammatory factor level and satisfaction were statistically analyzed between the two groups.Result:After treatment,the incidence interval of patients in the intensive group was longer than that in the routine group,the time for blood sugar to return to normal was shorter than that in the routine group (P<0.05).After treatment,FPG,HbA1c,TC and TG in the intensive group were lower than those in the routine group (P<0.05).After treatment,the levels of CI,PLI,BI,BOP,PD,CAL and IL-1β in the intensive group were lower than those in the routine group (P<0.05).After treatment,the levels of serum hs-CRP and TNF-α in the intensive group were lower than those in the routine group (P<0.05).The satisfaction of patients in the intensive group was 93.33%,which was higher than 73.33% in the routine group (P<0.05).Conclusion:In the treatment of patients with type 2 diabetes mellitus complicated with chronic periodontitis,intensive hypoglycemic therapy combined with periodontal basic therapy is more effective than conventional hypoglycemic therapy combined with periodontal basic therapy in controlling blood glucose and p
