摘　　要：目的:观察关节镜下直接松解联合肩袖关节修复术对肩袖损伤患者肌肉力量及肩关节功能的影响。方法:选取2018年2月-2020年3月九江市第一人民医院收治的104例肩袖损伤患者,按照随机数字表法将患者分为对照组和研究组,每组52例。对照组行手法松解联合肩袖关节修复术治疗,研究组行关节镜下直接松解联合肩袖关节修复术治疗,术后均随访6个月。比较两组围手术期指标、肌肉力量及肩关节功能(ASES评分)。结果:研究组手术耗时、术后疼痛减轻时间、总住院时间均短于对照组,差异均有统计学意义(P<0.05)。术后6个月,两组患肢肌肉力量均高于术前,且研究组高于对照组,差异均有统计学意义(P<0.05)。术后6个月,两组ASES评分均高于术前,且研究组高于对照组,差异均有统计学意义(P<0.05)。结论:肩袖损伤患者采用关节镜下直接松解联合肩袖关节修复术治疗可缩短手术时间,减轻术后疼痛,提高肌肉力量与肩关节功能,利于患者早期康复。Objective:To observe the effects of arthroscopic direct release combined with rotator cuff arthroplasty on muscle strength and shoulder function in patients with rotator cuff injury.Method:A total of 104 patients with rotator cuff injury admitted to Jiujiang First People’s Hospital from February 2018 to March 2020 were selected,they were divided into control group and study group according to random number table method,with 52 patients in each group.The control group was treated with manual release combined with rotator cuff arthroplasty,and the study group was treated with arthroscopic direct release combined with rotator cuff arthroplasty,all patients were followed up for 6 months.Perioperative indicators,muscle strength and shoulder function (ASES score) were compared between the two groups.Result:The operative time,postoperative pain relief time and total hospital stay time in the study group were shorter than those in the control group,the differences were statistically significant (P<0.05).6 months after surgery,the muscle strength of the affected limb in both groups were higher than those before surgery,and that in the study group was higher than that in the control group,the differences were statistically significant (P<0.05).6 months after surgery,ASES scores in both groups were higher than those before surgery,and that in the study group was higher than that in the control group,the differences were statistically significant (P<0.05).Conclusion:Arthroscopic direct release combined with rotator cuff arthroplasty for patients with rotator cuff injury can shorten the operation time,relieve postoperative pain,improve muscle strength and shoulder function,and benefit the early recovery of patients.
