拉贝洛尔联合硝苯地平对妊娠期高血压疾病患者凝血功能、妊娠结局的影响

摘　　要：目的:探究拉贝洛尔联合硝苯地平对妊娠期高血压疾病患者凝血功能及妊娠结局的影响。方法:选取鹰潭市人民医院妇产科2018年7月-2021年7月收治的妊娠期高血压疾病患者76例,随机分为常规组38例和联合组38例。常规组给予单一的硝苯地平进行治疗,联合组给予拉贝洛尔联合硝苯地平进行治疗。比较两组患者治疗前后血压值、血清同型半胱氨酸(Hcy)、胱抑素C水平、凝血功能及妊娠结局。结果:治疗后,两组收缩压和舒张压均低于本组治疗前,联合组均低于常规组(P<0.05);治疗后,联合组血清Hcy、胱抑素C水平均显著低于常规组(P<0.05);治疗后,联合组的纤维蛋白原(FIB)、D-二聚体(D-D)均低于常规组,凝血酶原时间(PT)长于常规组,差异均有统计学意义(P<0.05);联合组的剖宫产、胎盘早剥、新生儿窒息、早产、产后出血发生率均低于常规组(P<0.05)。结论:对妊娠期高血压疾病患者给予拉贝洛尔联合硝苯地平治疗,可以有效地降低患者的舒张压、收缩压,调节患者血清Hcy和胱抑素C水平,改善凝血功能,减少不良妊娠结局的发生,值得推广应用。Objective:To explore the effect of Labetalol combined with Nifedipine on coagulation function and pregnancy outcome in patients with hypertensive disorders of pregnancy.Method:A total of 76 patients with hypertensive disorders of pregnancy who were admitted to the Department of Obstetrics and Gynecology of Yingtan People’s Hospital from July 2018 to July 2021 were randomly divided into the routine group and the combined group,with 38 cases in each group.The routine group was treated with Nifedipine alone,and the combined group was treated with Labetalol combined with Nifedipine.The blood pressure,serum Hcy,cystatin C levels,coagulation function and pregnancy outcome were compared between the two groups before and after treatment.Result:After treatment,the systolic and diastolic blood pressures of the two groups were lower than those before treatment,and those in the combined group were lower than those in the routine group (P<0.05);after treatment,the levels of serum Hcy and cystatin C in the combined group were significantly lower than those in the routine group (P<0.05);after treatment,fibrinogen (FIB) and D-Dimer (D-D) in the combined group were lower than those in the routine group,and prothrombin time (PT) was higher than that in the routine group (P<0.05);the incidences of cesarean section,placental abruption,neonatal asphyxia,premature delivery and postpartum hemorrhage in the combined group were lower than those in the routine group (P<0.05).Conclusion:Labetolol combined with Nifedipine for patients with hypertensive disorders of pregnancy can effectively reduce the diastolic and systolic blood pressure,regulate the levels of serum Hcy and cystatin C,improve the coagulation function and reduce the occurrence of adverse pregnancy outcomes,it is worthy of popularization and application.
