髋臼骨折后创伤性关节炎行全髋关节表面置换术对患者并发症及生活质量的影响

摘　　要：目的:探讨髋臼骨折后创伤性关节炎行全髋关节表面置换术治疗的临床价值。方法:选择2019年8月-2020年9月佳木斯市中心医院收治的103例髋臼骨折手术并发创伤性关节炎患者,按随机数字表法将其分为对照组(n=51)与研究组(n=52)。对照组给予全髋关节置换术治疗,研究组给予全髋关节表面置换术治疗。比较两组临床效果、临床指标、并发症发生情况及治疗前后生活质量。结果:研究组优良率高于对照组(P<0.05);研究组治疗后3 d的VAS评分低于对照组,下床活动时间与住院时间均短于对照组,差异均有统计学意义(P<0.05)。研究组并发症发生率低于对照组(P<0.05)。研究组治疗后3个月各维度生活质量评分均高于对照组(P<0.05)。结论:全髋关节表面置换术治疗髋臼骨折后创伤性关节炎的效果较好,有助于减轻患者疼痛,缩短康复时间,提高患者生活质量水平,并发症发生率较低,值得推广应用。Objective:To explore the clinical value of total hip replacement for traumatic arthritis after acetabular fracture.Method:A total of 103 patients with traumatic arthritis after acetabular fracture admitted to Jiamusi City Central Hospital from August 2019 to September 2020 were selected.They were divided into the control group (n=51) and the study group (n=52) by random number method.The control group was given total hip replacement,and the study group was given total hip resurfacing arthroplasty.The clinical effect,clinical indicators,complications and quality of life before and after treatment were compared between the two groups.Result:The total excellent and good rate of the study group was higher than that of the control group (P<0.05).The VAS score of the study group at 3 d after treatment was lower than that of the control group,while getting out of bed and hospitalization time were less than those of the control group,the differences were statistically significant (P<0.05).The incidence of complications in the study group was lower than that in the control group (P<0.05).The scores of all dimensions of quality of life in the study group at 3 months after treatment were higher than those in the control group (P<0.05).Conclusion:Total hip resurfacing arthroplasty has a good effect in the treatment of traumatic arthritis after acetabular fracture,which is helpful to relieve the pain of patients,shorten the recovery time,improve the quality of life of patients,and has a low incidence of complications,therefore,it is worthy of popularization and application.
