摘　　要：目的研究全内技术重建前交叉韧带的临床效果。方法采用回顾性病例对照研究,选取2017年7月至2019年6月在长沙市中心医院接受全内技术重建前交叉韧带的20例为全内组,选取同期同组医师行自体腘绳肌单束重建的20例为对照组。排除膝关节外伤史、手术史、多发韧带损伤以及半月板撕裂者。比较两组的临床效果。结果所有患者均获随访,平均随访时间8(6~12)个月。两组患者伤口愈合良好,膝关节无明显肿胀。组内比较,术后6周及12周膝关节功能评分(Lysholm评分)、运动评分(Tegner评分)及VAS评分较术前均明显改善,有统计学差异(P=0.000)。组间比较,术后6周及12周Lysholm评分、Tegner评分及VAS均无明显差异(P>0.05)。结论全内技术重建前交叉韧带在症状改善及功能恢复方面与传统手术技术相似,都能获得满意的疗效,但全内技术的肌腱利用率高,手术方法更加安全可靠。Objective To study the clinical effects of all-inside anterior cruciate ligament(ACL)reconstruction.Methods A retrospective case-control study was conducted.20 patients who underwent all-inside anterior cruciate ligament reconstruction in Changsha Central Hospital from July 2017 to June 2019 were selected as the experimental group,and 20 patients who underwent single bundle reconstruction of autogenous hamstring muscle by doctors in the same group during the same period were selected as the control group.The patients with history of knee trauma,knee surgery,multiple ligament injuries or meniscus tear were excluded.The clinical effects of the two groups were compared.Results All patients were followed up for an average of 8(6~12)months.The wound healed well without obvious swelling of the knee joint.Lysholm score,Tegner score and VAS of the two groups at 6 and 12 weeks after operation improved significantly,compared with pre-operative ones(P=0.000).There was no significant difference in Lysholm score,Tegner score and VAS between the two groups at 6 and 12 weeks after operation(P>0.05).Conclusion All-inside ACL reconstruction and the traditional reconstruction both demonstrate good clinical and functional outcomes.But the all-inside ACL reconstruction may have a higher tendon utilization rate,and may be a safe and reliable procedure.
