健康教育结合情景模拟访视在初产妇中的应用效果

摘  要:目的探讨健康教育结合情景模拟访视在初产妇中的应用效果。方法选取2020年1月至2021年4月拟于华中科技大学同济医学院附属同济医院分娩的90名初产妇为研究对象,按随机数字表法分为对照组和观察组,各45名。对照组初产妇给予优质护理,观察组初产妇给予健康教育结合情景模拟访视。比较2组的分娩方式、母婴情况、分娩自我效能、心理状况及分娩不良事件。结果观察组的剖宫产率低于对照组,经阴道分娩率高于对照组(P<0.05)。观察组初产妇的产后2 h出血量低于对照组,总产程短于对照组(P均<0.05);2组间新生儿的1 min Apgar评分差异无统计学意义(P>0.05)。分娩前,2组的简化中文版分娩自我效能量表各维度评分均高于干预前(P均<0.05),且观察组均较对照组高(P均<0.05)。分娩结束时,2组的爱丁堡产后抑郁量表和焦虑自评量表评分均低于干预前(P均<0.05),且观察组均较对照组低(P均<0.05)。观察组的分娩不良事件总发生率低于对照组(P<0.05)。结论健康教育结合情景模拟访视干预模式可提高初产妇自然分娩率和分娩自我效能,改善其负性情绪和减少分娩不良事件的发生。Objective To explore the application effect of health education combined with situational simulation visits on primiparas.Methods A total of 90 primiparas who were scheduled to give birth in Tongji Hospital,Tongji Medical College,Huazhong University of Science and Technology from January 2020 to April 2021 were selected as the research objects and divided into the control group and the observation group by the use of random number table,each with 45 cases.The primiparas in control group were given high-quality nursing,while the primiparas in the observation group were given health education combined with situational simulation visits.The mode of delivery,maternal and infant conditions,self-efficacy of delivery,psychological status,and adverse events of delivery were compared between the two groups.Results The observation group had a lower cesarean section rate and a higher vaginal delivery rate than the control group(P<0.05).The amount of blood loss at 2 hr postpartum in the observation group was lower than that in the control group,and the duration of total labor was shorter than that in the control group(both P<0.05).There was no significant difference in the 1-min Apgar scores of neonates between the two groups(P>0.05).Before delivery,the scores of short form of the Chinese childbirth self-efficacy inventory in the two groups were higher than those before intervention(all P<0.05),and the scores of the observation group were higher than those of the control group(both P<0.05).At the end of delivery,the scores of Edinburgh postnatal depression scale and self-rating anxiety scale in the two groups were lower than those before intervention(all P<0.05),and the scores of the observation group were lower than those of the control group(both P<0.05).The observation group had a lower incidence of adverse events of delivery than the control group(P<0.05).Conclusion The intervention mode of health education combined with situational simulation visits can increase the spontaneous delivery rate of primiparas,improve the de
