膝关节置换手术支撑圆枕固定装置应用的比较研究

摘　　要：目的探讨膝关节置换手术支撑圆枕固定装置的应用价值。方法选择2019年1月~2020年5月我院同一手术团队对重度骨性关节炎行单侧膝关节置换术120例,按照入院时间先后顺序进行分组,前60例使用传统约束带方法固定支撑圆枕(对照组),后60例使用膝关节置换手术支撑圆枕固定装置固定支撑圆枕(研究组)。比较2组体位垫移位情况,肢体有效支撑评分,安置体位时间和医护人员的满意度。结果研究组肢体有效支撑评分(8.5±0.6)分,显著高于对照组(8.1±0.6)分(t=3.732,P=0.000);体位垫移位率由13.3%(8/60)下降到1.7%(1/60)(χ^(2)=4.324,P=0.038);安置体位时间由(2.4±0.6)min缩短至(2.1±0.3)min(t=-3.152,P=0.000);医生满意度由4(4,5)分提升至5(4,5)分(Z=-4.069,P=0.000);护士满意度由4(4,4)分提升至5(4,5)分(Z=-4.724,P=0.000)。结论使用膝关节置换手术支撑圆枕固定装置可控制支撑圆枕移位、提升患肢支撑效果、缩短体位摆放时间,提升医护人员对膝关节置换手术体位摆放的满意度。Objective To explore the application value of supporting round pillow fixation device for knee arthroplasty.Methods According to the chronological order,120 cases of unilateral knee arthroplasty for severe osteoarthritis,treated by the same surgical team in our hospital from January 2019 to May 2020,were divided into two groups.The control group was set up by using the traditional restraint band method to fix the supporting round pillow,and the study group was set up by using the knee arthroplasty supporting pillow fixing device to fix the supporting round pillow,with 60 cases in each group.The offset of supporting round pillow,effective support of limbs,placement time of body position and the satisfaction of medical staff were compared between the two groups.Results The score of effective supporting limb in the study group(8.5±0.6)points was significantly higher than that in the control group[(8.1±0.6)points,t=3.732,P=0.000].The displacement rate of body pad decreased from 13.3%(8/60)to 1.7%(1/60)(χ^(2)=4.324,P=0.038).The placement time of body position decreased from(2.4±0.6)min to(2.1±0.3)min(t=-3.152,P=0.000).The satisfaction of doctors increased from 4(4,5)points to 5(4,5)points(Z=-4.069,P=0.000),and the satisfaction of nurses increased from 4(4,4)points to 5(4,5)points(Z=-4.724,P=0.000).Conclusion The use of a supporting pillow fixation device for knee replacement can control the displacement of the pillow,improve the support effect of the affected limb,shorten the placement time of body position,and improve the satisfaction of medical staff.
