摘　　要：目的比较剑突下入路胸腔镜手术(S-VATS)与侧胸入路电视胸腔镜手术(I-VATS)同期行双侧肺大疱切除的近期疗效。方法检索中国知网、中国生物医学文献数据库、维普、万方、The Cochrane Library、Embase、PubMed等数据库,检索时间从建库至2021年9月,采用RevMan 5.4软件对数据进行定量分析,合并后进行meta分析。结果总共纳入7篇文献,总病例数为788例。meta分析结果显示:S-VATS组的手术时间、术中出血量、术后带管时间、术后24 h疼痛评分、术后并发症发生率较I-VATS均显著减少,差异有统计学意义(P<0.05)。两组的术后引流量差异无统计学意义(P>0.05)。结论现有证据表明,S-VATS可缩短手术时间、术后带管时间,减少术中出血量、术后并发症发生率、术后24 h疼痛评分,更有利于患者术后恢复。Objective To compare the short-term efficacy of simultaneous bilateral bullectomy with subxiphoid video-assisted thoracoscopic surgery(S-VATS)versus intercostal video-assisted thoracoscopic surgery(I-VATS).Methods Literature included in China National Knowledge Internet,China Biology Medicine disc,VIP and Wanfang databases,The Cochrane Library,Embase,PubMed and other databases from the establishment of the databases to September 2021 was retrieved,and the obtained data were quantitatively analyzed and combined using RevMan5.4 before meta-analysis was conducted.Results 7 literature were included in this study,with a total of 788 cases.The results of the meta-analysis showed that the surgical duration,intraoperative bleeding volume,postoperative catheter indwelling time,postoperative 24-hour pain score,and the incidence of postoperative complications in the S-VATS group were significantly shorter/lower than those in the I-VATS group,with statistically significant differences(P<0.05).There was no statistically significant difference between the two groups in postoperative drainage(P>0.05).Conclusion It has been proved that S-VATS can shorten the surgical duration and postoperative catheter indwelling time,reduce intraoperative bleeding volume and the incidence of postoperative complications,as well as lower the postoperative 24-hour pain score,which is more beneficial to p atients’postoperative recovery.
