摘　　要：目的探析肾筋膜扩张器在女性输尿管结石合并下段狭窄中的应用。方法选取2019年10月至2020年12月我院行输尿管镜钬激光碎石术中发现的输尿管下段狭窄女性患者50例,随机数字表法分为对照组和观察组各25例,对照组采用传统一次性输尿管导管扩张治疗,观察组采用肾筋膜扩张器治疗。比较两组患者手术指标、术后疼痛情况及并发症发生情况。结果观察组双J管留置时间、手术时间、住院时间均短于对照组(P<0.05);观察组的住院费用少于对照组(P<0.05);观察组术后12 h、24 h、48 h时段的疼痛视觉模拟评分(VAS)均低于对照组(P<0.05);观察组并发症发生率为4.0%,低于对照组的44.0%(P<0.05)。结论女性输尿管下段狭窄使用肾筋膜扩张器行狭窄扩张,能够缩短双J管留置时间,减少患者术后疼痛程度,缩短患者住院时间并减少治疗费用,并发症少,值得临床推广使用。Objective To explore the application of renal fascia dilator in female ureteral calculi complicated with lower segment stenosis.Methods We Select 50 female patients diagnosed with lower ureteral stricture during ureteroscopic holmium laser lithotripsy in our hospital from October 2019 to December 2020,and divide them into observation group and control group randomly,with 25 cases in each group.The control group was treated with traditional disposable ureteral catheter dilatation,and the observation group was treated with renal fascia dilator.Surgical indexes,postoperative pain and complications were compared between the two groups.Results The indwelling timeof the double J tube,operation time and hospitalization time in the observation group were shorter than those in the control group(P<0.05).The hospitalization expenses in the observation group were less than those in the control group(P<0.05).The visual analogue scale(VAS)of pain at 12 h,24 h,and 48 h after operationin the observation group was lower than that of the control group(P<0.05).The complication rate of the observation group was 4.0%,lower than the 44%of the control group(P<0.05).Conclusion The use of renal fascia dilator for the expansion offemale lower ureteral stenosis can shorten the indwelling time of double-J tube,reduce the postoperative pain,shorten the hospitalization time,reduce the cost of treatment,reduce the rate of complications,which is worthy of clinical promotion.
