50~70岁主动脉瓣病变患者生物瓣与机械瓣置换术后生活质量的对比研究

摘　　要：目的比较50~70岁主动脉生物瓣和机械瓣膜置换术后患者的生活质量和焦虑状况。方法回顾性分析2019年7月至2020年5月期间在遵义医科大学附属医院心外科行主动脉瓣置换术的76例(生物瓣组32例,机械瓣组44例)50~70岁患者的临床资料,患者均在出院时和1年随访时完成SF-36量表,并在1年随访时完成心脏焦虑问卷。结果出院时生物瓣膜组和机械瓣膜组的SF-36评分差异无统计学意义(P>0.05);而随访1年时生物瓣膜组SF-36评分大于机械瓣膜组(P<0.05);机械瓣膜组心脏焦虑问卷评分大于生物瓣膜组(P<0.05)。结论50~70岁主动脉生物瓣置换术后患者的SF-36生活质量评分高于机械瓣膜置换术后的患者,而心脏焦虑问卷的评分与之相反,这类患者在选择心脏瓣膜的时候,可以考虑使用生物瓣膜。Objective To compare the quality of life and anxiety of 50~70 years old patients who underwent aortic biological vs mechanical valve replacement.Methods Between July 2019 and May 2020,76(50~70)years old patients underwent aortic valve replacement in Affiliated Hospital of Zunyi Medical University,clinical data of these patients were analyzed retrospectively(32 cases with bioprostheses vs 44 cases with mechanical prostheses).The SF-36 form was completed by all patients at discharge and at one-year follow-up,and the cardiac anxiety questionnaire was also completed at one-year follow-up.Results There was no significant difference in SF-36 scores between the biological valve group and the mechanical valve group at discharge(P>0.05).However,at one-year follow-up,the SF-36 scores were significantly higher in the biological valve group than in the mechanical valve group(P<0.05),and the cardiac anxiety questionnaire scores in the mechanical valve group were significantly higher than those in the biological valve group(P<0.05).Conclusion The SF-36 quality of life score of patients aged 50 to 70 years after biological valve replacement was higher than that of patients after mechanical valve replacement,while the score on the cardiac anxiety questionnaire was opposite.Biological valves should be considered when choosing heart valves in these patients.
