类风湿关节炎指伸肌腱自发性断裂超声与MRI对比研究

摘  要:目的:比较动态高频超声和磁共振成像对类风湿关节炎自发性肌腱断裂的诊断价值。方法:选取经手术确诊的类风湿关节炎指伸肌腱断裂患者16例,术前行超声及MRI检查。选取同期类风湿性关节炎手指背伸困难患者27例,比较2组患者手指滑膜厚度、滑膜血流分级、骨质破坏程度。结果:高频超声与MRI诊断指伸肌腱断裂无统计学差异,2种影像学检查诊断的特异度、灵敏度相当。断裂组滑膜血流分级及骨破坏程度高于未断裂组。结论:滑膜炎和骨侵蚀可能是类风湿关节炎指伸肌腱自发性断裂的关键因素。高频超声和MRI检测肌腱自发性断裂的诊断价值相当,可作为临床诊断的有效手段。Objective:To compare the diagnostic value of dynamic high frequency ultrasound and magnetic resonance imaging(MRI)in spontaneous tendon rupture of rheumatoid arthritis.Methods:Sixteen patients with rheumatoid arthritis diagnosed with rupture of finger extensor tendon were selected for ultrasound and MRI examination before operation.Twenty-seven patients with rheumatoid arthritis who had difficulty with finger extension in the same period were selected,and the thickness of the synovium of the fingers,the blood flow grade of the synovium,and the degree of bone destruction were compared between the two groups.Results:There is no statistical difference between high-frequency ultrasound and MRI in the diagnosis of extensor tendon rupture,and the specificity and sensitivity of the two imaging tests are equivalent.Synovial blood flow grading and bone destruction in the fractured group were higher than those in the unruptured group.Conclusion:Synovitis and bone erosion may be the key factors for spontaneous rupture of extensor tendon in rheumatoid arthritis.The diagnostic value of high-frequency ultrasound and MRI in detecting spontaneous rupture of tendon is equivalent and can be used as an effective means of clinical diagnosis.
