摘  要:目的探讨宽体探测器CT应用双低扫描模式行一站式冠状动脉CT血管造影(computed tomography angiography,CTA)和头颈部CTA扫描对患者辐射剂量及图像质量的影响。方法收集2019年10月~2020年9月首都医科大学宣武医院放射与核医学科行冠状动脉CTA和头颈部CTA一站式扫描患者60例,根据扫描条件及碘造影剂浓度分为常规组30例和双低组30例。常规组冠状动脉管电压为120 kVp,头颈部为100 kVp,碘造影剂浓度为370 mgI/ml;双低组冠状动脉管电压为100 kVp,头颈部为100 kVp,碘造影剂浓度为320 mgI/ml。由两名高年资放射科诊断医师对患者心脑血管图像质量进行主观评分,比较2组目标血管信噪比、对比噪声比。记录患者剂量报告生成的容积CT剂量指数、剂量长度乘积值,计算并比较2组有效辐射剂量。结果双低组主动脉根部对比噪声比明显低于常规组,左前降支信噪比明显高于常规组,差异有统计学意义(P<0.05,P<0.01)。双低组冠状动脉容积CT剂量指数、剂量长度乘积和有效辐射剂量明显低于常规组[(16.5±3.2)mGy vs(18.6±4.1)mGy,(265.3±45.8)mGy×cm vs(302.2±64.6)mGy×cm,(3.7±0.6)mSv vs(4.2±0.9)mSv,P<0.05]。结论宽体探测器CT一站式冠状动脉CTA和头颈部CTA扫描中应用双低扫描模式是可行的,图像质量和辐射剂量满足临床要求,具有广泛应用价值。Objective To study the effect of one-step low-tube voltage and low-iodine cervicocephalic and coronary CTA on radiation dose and image quality.Methods Sixty patients who underwent one-step low-tube voltage and low-iodine cervicocephalic and coronary CTA in our hospital from October 2019 to September 2020 were divided into conventional CTA group(n=30)and low-tube voltage and low-iodine CTA group(n=30).The tube voltage for coronary CTA and cervicocephalic CTA was 120 kVp and 100 kVp respectively and the concentration of iodine contrast was 370 mgI/ml and 320 mgI/ml respectively in low-tube voltage and low-iodine CTA group.The image quality was subjectively scored by two senior radiologists.The signal-to noise-ratio(SNR)and contrast-to-noise ratio(CNR)of targeted blood vessels were calculated and compared between the two groups.The CT dose repot-generated volume CT dose index and dose-length product(DLP)were recorded.The effective radiation dose was calculated and compared between the two groups.Results The CNR at aortic root was lower and the SNR of left anterior descending coronary artery was significantly higher in low-tube voltage and low-iodine CTA group than in conventional CTA group(P<0.05,P<0.01).The coronary artery volume CT dose index,DLP and effective radiation dose were significantly lower in low-tube voltage and low-iodine CTA group than in conventional CTA group(16.5±3.2 mGy vs 18.6±4.1 mGy,265.3±45.8 mGy×cm vs 302.2±64.6 mGy×cm,3.7±0.6 mSv vs 4.2±0.9 mSv,P<0.05).Conclusion One-step low-tube voltage and low-iodine coronary CTA and cervicocephalic CTA can improve the image quali-ty,reduce the radiation dose and is of wide application value in clinical practice.
