经耳内镜鼓室注射地塞米松治疗梅尼埃病的分析

摘　　要：目的 分析经耳内窥镜鼓室内注射地塞米松治疗梅尼埃病的方法和疗效。方法 在耳内窥镜下对20例单侧梅尼埃病患者行固定头位下鼓室内注射地塞米松,比较患者注药前后眩晕、听力及耳鸣变化情况。结果 20例均随访18个月,眩晕控制A 级(完全控制)13 例,B 级(基本控制)5例,C 级(部分控制)2 例;10 例治疗后听力有好转,9例无变化,听力继续下降者1例;耳鸣有9例完全消失,7 例好转,4 例无明显改变。鼓室内注射地塞米松后,鼓膜穿刺孔在1个月内全部愈合。结论 经耳内镜鼓室内精准注射地塞米松可有效控制梅尼埃病的眩晕症状,改善听力和耳鸣。Objective To analysis the ways and effect of the intratympanic dexamethasone injection for Meniere′s disease.Methods Twenty patients with unilateral Meniere′s disease were injected with dexamethasone in the tympanum under ear endoscope.Results Twenty patients were all followed up for 18 months.In vertigo control,there were 13 cases of grade A (complete control),5 cases of grade B (basic control) and 2 cases of grade C (partial control).The hearing improved in 10 cases,remained unchanged in 9 cases,and continued hearing loss in 1 case.Tinnitus disappeared completely in 9 cases,improved in 7 cases and remained unchanged in 4 cases.The perforated tympanic membrane all healed completely within 1 month after injection.Conclusion The intratympanic injection of dexamethasone can effectively control the symptoms of vertigo and improve the hearing and tinnitus in Meniere’s disease.
