摘 要:目的:探讨校正MEWS的信息化病情预警法在重症肺炎(SP)患者中的应用效果。方法:选择2019年1月1日~2021年1月1日收治的160例SP患者,按照床位号单双数分为对照组78例和观察组82例,对照组采用常规护理,观察组在对照组基础上采用校正MEWS的信息化病情预警法;比较两组干预前后疾病控制情况[采用校正MEWS评分、临床肺部感染评分(CPIS)]、健康水平[采用急性生理及慢性健康状况评分系统Ⅱ(APACHEⅡ)]、并发症(包括感染性休克、肺水肿、急性呼吸窘迫综合征)和疾病结局。结果:干预后,观察组校正MEWS、CPIS、APACHEⅡ各维度评分均低于对照组(P<0.01);观察组并发症发生率低于对照组(P<0.01);观察组干预有效率高于对照组(P<0.01)。结论:校正MEWS的信息化病情预警法能够改善SP患者疾病危急程度,降低肺部感染率,减少感染性休克、肺水肿和急性呼吸窘迫综合征等并发症的发生,同时改善患者预后。Objective:To explore the application effect of information-based disease early warning method based on Corrected Modified Early Warning Score(MEWS)in patients with severe pneumonia.Methods:A total of 160 patients with severe pneumonia admitted during January 1,2019 to January 1,2021 were divided into the control group with 78 cases and the observation group with 82 cases according to the number table method.The control group received routine nursing,and the observation group was given information-based disease early warning method based on corrected MEWS score for intervention on the basis of the control group.Corrected MEWS score,Clinical Pulmonary Infection Score(CPIS),Acute Physiology and Chronic Health EvaluationⅡ(APACHEⅡ)were used to assess the disease control and health level of patients with severe pneumonia,as well as changes in incidence rates of complications(septic shock,pulmonary edema,acute respiratory distress syndrome)and disease outcomes were compared between the two groups before and after intervention.Results:After intervention,the corrected MEWS scores,CPIS scores and the scores of dimensions of APACHEⅡin the observation group were lower than those in the control group(P<0.01).The incidence rate of complications in the observation group was lower than that in the control group(P<0.01),and the effective rate in the observation group was higher than that in the control group(P<0.01).Conclusion:Information-based disease early warning method based on corrected MEWS score can improve the disease criticality in SP patients,reduce the pulmonary infection,lower the occurrence of septic shock,pulmonary edema and acute respiratory distress syndrome caused and improve the prognosis of patients.
