持续冷疗在高强度超声消融治疗中减轻骶尾部疼痛的临床效果研究

摘　　要：目的:探讨持续冷疗干预在高强度超声消融(HIFU)治疗中减轻骶尾部疼痛的临床效果研究。方法:选择2020年9月1日~2021年3月1日收治的100例HIFU治疗子宫肌瘤患者,采用完全随机分组将患者分为对照组和观察组各50例,对照组采用常规镇痛干预,观察组在常规镇痛干预基础上给予持续冷疗干预;比较两组因疼痛引起的暂停次数、暂停时间、围术期基本情况(手术时间、下床活动时间、住院时间)、疼痛情况[采用视觉模拟评分法(VAS)]、焦虑[采用汉密顿焦虑量表(HAMA)]、舒适度[采用舒适状况量表(GCQ)]及术后满意度。结果:观察组因疼痛引起的暂停次数、暂停时间少于对照组(P<0.01);观察组手术时间、下床活动时间、住院时间均短于对照组(P<0.05);观察组术中、术后2 h、术后6 h疼痛程度均低于对照组(P<0.01);术后1 d,两组HAMA评分低于术前(P<0.05),且观察组低于对照组(P<0.01);两组GCQ评分高于术前(P<0.05),且观察组高于对照组(P<0.01);观察组术后满意度高于对照组(P<0.01)。结论:在HIFU治疗中持续冷疗干预能减轻子宫肌瘤患者骶尾部疼痛,加快患者术后恢复,缓解焦虑,提高舒适度及满意度。Objective:To explore the clinical effect of continuous cryotherapy intervention on reducing sacrococcygeal pain during high-intensity focused ultrasound ablation(HIFU).Methods:A total of 100 patients with uterine fibroids admitted by HIFU during September 1,2020 to March 1,2021 were selected,and were divided into the control group and the observation group each with 50 cases by the completely randomized grouping.The control group was given routine analgesia intervention,and the observation group was given continuous cryotherapy intervention on the basis of routine analgesia.The pauses frequency caused by pain,duration of pauses and basic perioperative situation(surgical time,ambulation time,hospital stay)were compared between the two groups.Pain status by Visual Analogue Score(VAS),anxiety degree by Hamilton Anxiety Scale(HAMA)and comfort by General Comfort Questionnaire(GCQ)and postoperative satisfaction between the two groups were also compared.Results:The pauses frequency and duration of pauses caused by pain in the observation group were less than those in the control group(P<0.01).The surgical time,ambulation time and hospital stay in the observation group were lower than those in the control group(P<0.05).The pain degree of patients in the observation group was lower than that in the control group during surgery and at 2 h and 6 h after surgery(P<0.01).At 1 d after surgery,the HAMA scores in the two groups were lower than those before surgery(P<0.05),and lower in the observation group than those in control group(P<0.01).The GCQ scores in the two groups were higher than those before surgery(P<0.05),and higher in the observation group than those in the control group(P<0.01).The postoperative satisfaction in the observation group was higher than that in the control group(P<0.01).Conclusion:Continuous cryotherapy intervention in HIFU treatment can reduce the sacrococcygeal pain,speed up the postoperative recovery,relieve the anxiety,and improve the comfort and satisfaction,and it has a high clinical application
