单枚椎间融合器加单边椎弓根内固定治疗老年腰椎退变性疾病疗效对比

摘　　要：目的比较单枚椎间融合器加单边和双边椎弓根内固定治疗单节段腰椎退变性疾病的临床疗效和影像学改变。方法回顾性研究2015年5月—2017年12月36例单节段腰椎退变并接受单枚椎间融合器加单边和双边椎弓根内固定患者,依据手术方式不同分为两组,单边组(19例,单边椎弓根内固定加椎间融合术治疗)和双边组(17例,双边椎弓根内固定加椎间融合术治疗),比较两组患者手术时间、出血量及住院费用,运用Oswestry指数评估患者手术前后的临床效果。结果单边组手术时间、出血量和内置物费用明显少于双边组,差异有统计学意义(P<0.05)。单边组内固定术后3次ODI指数(18.21±4.02)分、(15.21±3.13)分、(14.71±0.42)分均优于双边组,差异有统计学意义(t=4.206、4.169、14.296,P<0.05)。结论单枚椎间融合器加单边椎弓根内固定治疗腰椎退变性疾病,可显著缩减手术时间、降低患者出血量,可以达到较满意的临床效果。Objective To compare the clinical efficacy and imaging changes of a single interbody fusion cage plus unilateral and bilateral pedicle internal fixation in the treatment of single-segment lumbar degenerative diseases.Methods From May 2015 to December 2017,36 patients with single-segment lumbar spine degeneration who received a single interbody cage plus unilat⁃eral and bilateral pedicle internal fixation were retrospectively studied.They were divided into two groups according to different surgical methods.The unilateral group(19 cases,treated with unilateral pedicle internal fixation plus interbody fusion)and the bilateral group(17 cases,treated with bilateral pedicle fixation plus interbody fusion),the operation time and blood loss and hospitalization costs were compared between the two groups,and the Oswestry index was used to evaluate the clinical effect of patients before and after surgery.Results The operation time,blood loss and cost of implants in the unilateral group were sig⁃nificantly less than those in the bilateral group,and the differences were statistically significant(P<0.05).The unilateral group had better ODI scores(18.21±4.02)points,(15.21±3.13)points and(14.71±0.42)points after internal fixation than the bilateral group,and the differences were statistically significant(t=4.206,4.169,14.296,P<0.05).Conclusion A single intervertebral fu⁃sion cage and unilateral pedicle internal fixation for the treatment of degenerative lumbar spine diseases can significantly re⁃duce the operation time,reduce the patient's bleeding,and achieve satisfactory clinical results.
