摘 要:目的:探讨术前下肢全长CT相关参数分析在初次全膝关节置换术中的应用价值。方法:本研究选取的起始时间为2020年1月,截止时间为2021年6月,选取对象为40例初次行全膝关节置换术的患者。根据术前是否实施下肢全长CT相关参数分析将这些患者分为对照组和观察组,每组各有患者20例。术前对观察组患者实施下肢全长CT相关参数分析,术前未对对照组患者实施下肢全长CT相关参数分析。比较两组患者手术前后的影像学指标及膝关节功能评分表(KSS)的评分。结果:术前,两组患者的髋膝踝角、股胫角、冠状位股骨角、冠状位胫骨角相比,P>0.05。术后,两组患者的髋膝踝角、股胫角、冠状位股骨角和冠状位胫骨角均有所改善,均处于正常偏差范围之内,组内比较,P<0.05。术后,两组患者的髋膝踝角、股胫角、冠状位股骨角、冠状位胫骨角相比,P>0.05。术前,两组患者的KSS评分相比,P>0.05。术后,两组患者的KSS评分均高于术前,P<0.05。术后,观察组患者的KSS评分高于对照组患者,P<0.05。结论:在患者初次行全膝关节置换术前,对其实施下肢全长CT相关参数分析有助于准确定位其下肢机械力线,提高手术操作的精准度,促进其术后膝关节功能的恢复。Objective:To investigate the application value of preoperative full-length CT of lower limbs in primary total knee replacement.Methods:Starting from January 2020 to June 2021,40 patients who underwent total knee replacement for the first time were selected for this study.These patients were divided into control group and observation group,with 20 patients in each group,according to whether the preoperative analysis of parameters related to full-length CT of lower limbs was performed.Preoperative analysis of parameters related to full-length CT of lower limbs was performed for patients in the observation group,but not for patients in the control group.Imaging indexes and knee function Rating Scale(KSS)scores were compared between the two groups before and after surgery.Results:Before surgery,the comparison of hip knee and ankle Angle,femoral tibial Angle,coronal femoral Angle and coronal tibial Angle between the two groups was P>0.05.After surgery,hip knee and ankle Angle,femoral tibial Angle,coronal femoral Angle and coronal tibial Angle of 2 groups were improved,all within the normal deviation range,intra-group comparison,P<0.05.After surgery,the comparison of hip knee and ankle Angle,femoral tibial Angle,coronal femoral Angle and coronal tibial Angle between the two groups showed P>0.05.Before surgery,the KSS score of the two groups was compared,P>0.05.After surgery,KSS score of 2 groups was higher than that before surgery,P<0.05.After operation,KSS score of observation group was higher than control group,P<0.05.Conclusions:Before total knee replacement for the first time,the analysis of related parameters of lower limb full-length CT is helpful to accurately locate the mechanical force line of the lower limb,improve the accuracy of surgical operation,and promote the recovery of postoperative knee funct ion.
