腰脊神经后支阻滞联合体外冲击波治疗腰椎小关节源性腰痛的疗效

摘　　要：目的 观察腰脊神经后支阻滞联合体外冲击波疗法治疗腰椎小关节源性腰痛的临床效果。方法 84例腰椎小关节源性腰痛患者随机分为两组,每组42例。A组采用腰脊神经后支阻滞联合体外冲击波疗法治疗,B组单用腰脊神经后支阻滞作为对照。比较两组疗效。结果 A组总有效率95.24%,优于B组的78.57%(P<0.05)。治疗后,两组VAS疼痛评分、Oswestry功能障碍指数评分和日本骨科协会评估治疗分数均较治疗前降低,生活质量评分均较治疗前升高(P<0.05)。治疗后,A组的上述指标均优于B组(P<0.05或P<0.01)。A组治疗满意度高于B组(97.62%vs.83.33%)(P<0.05)。结论 与单用腰脊神经后支阻滞比较,联合体外冲击波治疗腰椎小关节源性腰痛疗效更好。Objective To observe the efficacy of the posterior branch block of the lumbar spine nerve combined with in vitro shock wave therapy in treating lumbar facet-derived low back pain.Methods A total of 84 patients with lumbar facet-derived low back pain were divided into two groups with 42 cases each.Group A was treated with the posterior branch block of the lumbar spine nerve combined with in vitro shock wave therapy.Group B was treated with the posterior branch block of the lumbar spine nerve alone as the control.The therapeutic efficacy was compared between the two groups.Results The total effectiveness rate of group A was 95.24%,which was higher than 78.57% of group B(P<0.05).Compared with before treatment, the VAS pain scores, ODI scores and JOA scores were significantly lower and scores of quality of life were increased after treatment in two groups(P<0.05),which were better in group A than those in group B(P<0.05 or P<0.01).Treatment satisfaction of group A was significantly higher than that of group B(97.62% vs.83.33%)(P<0.05).ConclusionThe efficacy of the posterior branch block of the lumbar spine nerve combined with in vitro shock wave therapy is better than the posterior branch block of the lumbar spine nerve alone in the treatment of the patients with lumbar facet-derived low back pain.
