电针对绝经后骨质疏松症模型大鼠肠黏膜维生素D膜相关性快速反应类固醇结合蛋白表达的影响

摘　　要：目的通过观察去卵巢骨质疏松模型大鼠肠黏膜1,25维生素D_(3)膜相关性快速反应类固醇结合(membrane-associated rapid response steroid-binding,1,25D_(3)-MARRS)蛋白表达,以及血清甲状旁腺激素相关蛋白(parathyroid hormone-related protein,PTHrP)、1,25-二羟基维生素D_(3)[1,25(OH)_(2)D_(3)]、骨保护素(osteoprotegerin,OPG)和核因子κB受体活化因子配体(receptor activator of nuclear factorκB ligand,RANKL)的变化,探讨电针治疗绝经后骨质疏松症的作用机制。方法将32只3月龄SD雌性大鼠随机分为空白组、假手术组、模型组及电针组,每组8只。模型组和电针组采用切除大鼠双侧卵巢的方法制备绝经后骨质疏松症模型,假手术组仅切除卵巢周围少量脂肪组织,空白组不做任何处理;造模后电针组进行电针干预。各组大鼠均进行骨密度检测,血清钙、PTHrP、1,25(OH)_(2)D_(3)、OPG、RANKL检测,以及肠黏膜1,25D_(3)-MARRS蛋白的表达检测。结果与空白组和假手术组比较,模型组大鼠股骨、胫骨骨密度明显下降(P<0.05)。与空白组和假手术组比较,模型组大鼠血清钙、PTHrP及1,25(OH)_(2)D_(3)水平明显降低(P<0.05,P<0.01),OPG/RANKL比值明显降低(P<0.01),且肠黏膜1,25D_(3)-MARRS蛋白表达减少(P<0.01)。与模型组比较,电针组股骨、胫骨密度升高(P<0.05,P<0.01),血清钙、PTHrP、1,25(OH)_(2)D_(3)水平明显升高(P<0.05,P<0.01),OPG/RANKL比值明显升高(P<0.01),肠黏膜1,25D_(3)-MARRS蛋白表达明显增加(P<0.05)。结论电针可通过提高血清钙、PTHrP及1,25(OH)_(2)D_(3)含量,提高OPG/RANKL比值,增加肠黏膜1,25D_(3)-MARRS蛋白表达,从而改善肠道钙吸收,提高去卵巢骨质疏松症大鼠的骨密度,最终达到防治骨质疏松的作用。Objective To discuss the mechanism of action of electroacupuncture(EA)in treating postmenopausal osteoporosis(PMOP)by observing the expression of 1,25 Vitamin D_(3) membrane-associated rapid response steroid(1,25D_(3)-MARRS)-binding protein and the changes in serum parathyroid hormone-related protein(PTHrP),1,25-dihydroxyvitamin D_(3)[1,25-(OH)_(2)D_(3)],osteoprotegerin(OPG),and receptor activator of nuclear factorκB ligand(RANKL).Method Thirty-two 3-month-old female Sprague-Dawley(SD)rats were randomized into a blank group,a sham operation group,a model group,and an EA group,with 8 rats in each group.The model and EA groups received bilateral ovariectomy to induce a PMOP model;in the sham operation group,only some fat tissues around the ovaries were removed;the blank group did not receive any intervention.EA intervention was offered to the EA group after modeling.The bone mineral density(BMD)was tested for each group as well as the serum calcium,PTHrP,1,25(OH)_(2)D_(3),OPG,and RANKL,and the expression of 1,25D_(3)-MARRS-binding protein in the intestinal mucosa.Result Compared with the blank and sham operation groups,the rat femoral and tibial BMD dropped significantly in the model group(P<0.05).Compared with the blank group and the sham operation group,the levels of serum calcium,PTHrP,and 1,25(OH)_(2)D_(3) dropped markedly(P<0.05,P<0.01),OPG/RANKL declined markedly(P<0.01),and the expression of 1,25D_(3)-MARRS-binding protein in the intestinal mucosa also declined significantly(P<0.01)in the model group.Compared with the model group,the femoral and tibial BMD increased(P<0.05,P<0.01),the levels of serum calcium,PTHrP,and 1,25(OH)_(2)D_(3) rose significantly(P<0.05,P<0.01),OPG/RANKL elevated notably(P<0.01),and the 1,25D_(3)-MARRS-binding protein in the intestinal mucosa increased markedly(P<0.05)in the EA group.Conclusion EA can improve intestinal calcium absorption and BMD in ovariectomy-induced PMOP rats via elevating the contents of serum calcium,PTHrP,and 1,25(OH)_(2)D_(3),OPG/RANKL,and the expression of 1
