局部麻醉全关节镜下腘窝囊肿摘除联合膝关节清理术的临床应用效果分析

摘　　要：目的:评价腘窝囊肿治疗中,局部麻醉全关节镜下摘除+膝关节清理术的重要价值。方法:选择我院2017年9月-2020年10月收治的62例腘窝囊肿患者参与研究,平均分组后,31例/组,分别行持续硬膜外麻醉全关节镜下腘窝囊肿摘除治疗(对照组)、局部麻醉全关节镜下摘除腘窝囊肿+膝关节清理术(观察组),比较2组患者并发症及IL-6、CKP指标水平。结果:手术后,组间并发症总发生率比,较对照组,观察组低(P>0.05),手术后、术后3天,组间IL-6、CRP指标水平比,较对照组,观察组水平均较低(P<0.05);2组膝关节屈伸活动度相比,术前差异不大(P<0.05);术后1天、2天观察组屈膝活动度大于对照组,伸膝活动度小于对照组,组间数据有统计学意义(P<0.05);2组术后1天、术后2天步行距离和VAS评分相比,观察组分别长于和低于对照组,组间数据有统计学意义(P<0.05);术前2组大腿周径相比,差异不大(P>0.05),术后1天、术后3天、出院时观察组大腿周径明显小于对照组,组间数据有统计学意义(P<0.05);SF-36评分2组相比,术前2组差异不大(P>0.05),术后观察组生活质量评分高于对照组,组间差异明显,组间数据有统计学意义(P<0.05)。结论:腘窝囊肿手术治疗中,局部麻醉全关节镜下摘除有其不可替代的优势,加以膝关节清理术,更能降低术后并发症,改善IL-6、CRP等指标,改善患者预后,值得应用。Objective:To evaluate the important value of arthroscopic excision+knee joint debridement under local anesthesia in the treatment of popliteal cysts.Methods:62 patients with popliteal cysts were taken to participate in the study.After the average grouping,31 cases/groups were treated with continuous epidural anesthesia under total arthroscopy(control group)and local anesthesia under total arthroscopy.Popliteal cyst+knee joint debride-ment(observation group),compare patients"complications and IL-6 and CRP index levels.Results:After surgery,the total incidence of complications between the groups was lower than that of the control group and the observation group(P<0.05);after surgery and 3 days after surgery,the IL-6 and CRP index levels between the groups were higher than those of the control group.The observation group had a lower level(P<0.05).Compared with the knee flexion and extension mobility in the two groups,Litde preoperative difference(P<0.05);At Id and 2d,the observation group had knee flexion mobility greater than the control group,Knee extension activity was less than that in the control group,Data were statistically significant between the groups(P<0.05);Walking distance between Id and 2d and YAS score between the two groups,The observation group was shorter than/lower than the control group,Data were statistically significant between the groups(P<0.05);Compared with the peripheral thigh diameter of the two experimental groups before surgery,The difference is small(P>0.05);The perithigh diameter of the observation group Id and 3d after the discharge was significantly smaller than that of the control group,Data were statistically significant between the groups(P<0.05);The SF-36 score,when compared to the two groups,Little preoperative difference between the two groups(P>0.05),The quality of life score was higher in the postoperative observation group than in the control group,The differences between the groups were obvious,Data between groups were statistically significant between groups(P<0.05).Conclu
