摘　　要：目的研究传染性单核细胞增多症(IM)合并肝功能损害患儿CD8^(+)CD28^(-)的变化特点,为临床治疗、预后判断提供依据。方法收集2016年1月至2021年10月该院儿科住院的525例IM患儿的临床资料,根据有无肝功能损害分为肝功能损害组(266例)和肝功能未损害组(259例)。对2组患儿的临床表现、异常淋巴细胞、尿常规、CD8^(+)CD28^(-)在内的T淋巴细胞亚群进行统计分析。结果肝功能损害组患儿年龄、眼睑水肿、肝脏肿大、异常淋巴细胞百分比大于10%检出率,以及CD3百分比、CD8百分比、CD8^(+)CD28^(-)计数及百分比均明显高于肝功能未损害组,差异均有统计学意义(P<0.05);轻、中度肝功能损害患儿各指标比较,差异均无统计学意义(P>0.05)。结论CD8^(+)CD28^(-)可能参与了IM患儿肝功能损害免疫过程,且发挥着保护作用。Objective To study the characteristics of CD8^(+)CD28^(-) changes in children with infectious mononucleosis(IM)combined with hepatic impairment,so as to provide a basis for clinical treatment and prognosis judgment.Methods The data of 525 children with IM who were hospitalized in pediatrics department of the hospital during January 2016 to October 2021 were collected.The children were divided into the liver function damage group(266 cases)and the liver function intact group(259 cases)according to the presence or absence of liver function damage.The clinical features,abnormal lymphocyte,urine routine and T lymphocyte subsets including CD8^(+)CD28^(-) were statistically analyzed in the two groups.Results The age,eyelid oedema,liver enlargement,the detection rate of abnormal lymphocyte percentage greater than 10%,CD3 percentage,CD8 percentage,CD8^(+)CD28^(-) count and percentage of children in the liver function damage group were significantly higher than those in the liver function intact group(P<0.05).The differences were no statistically significant(P>0.05)when comparing all indicators in children with mild and moderate liver function impairment.Conclusion CD8^(+)CD28^(-) may be involved in the immune process of liver function impairment in children with IM and play a protective role.
