胸椎旁神经阻滞与竖脊肌平面阻滞用于胸科术后患者镇痛及不良反应的Meta分析

摘　　要：目的 利用Meta分析方法综合评价比较胸椎旁神经阻滞与竖脊肌平面阻滞应用于胸科手术中术后镇痛以及不良反应效果影响Meta分析。方法 采用计算机检索MedLine、OVID、EMBASE、CNKI、Cochrane图书馆、PubMed、万方,无语种和发表时间限制。收集随机对照试验关于两种阻滞方式对患者术后镇痛影响。评价的指标包括:术后24 h静息时、咳嗽时VAS评分;术后48 h静息时、咳嗽时的VAS评分,采用了RevMan 5.2软件对数据进行Meta分析。结果 纳入共7项随机对照试验,共362例。与胸椎旁神经阻滞组相比,竖脊肌平面阻滞组患者于24 h静息时、咳嗽时VAS评分;术后48 h静息时、咳嗽时VAS评分差异无统计学意义(P> 0.05)。结论 超声引导下胸椎旁神经阻滞与竖脊肌平面神经阻滞对于胸科手术患者术后VAS评分不具统计学差异。Objective To comprehensively evaluate and compare the effects of paravertebral nerve block and vertical spinal muscle plane block on postoperative analgesia and adverse reactions in thoracic surgery through Meta analysis. Methods MEDLINE, Cochrane Library, EMBASE, Pub Med, Ovid, CNKI and Wanfang were searched by computer, without language and publication time limit.Randomized controlled trials were collected on the effects of two block methods on postoperative analgesia. The evaluation indexes included VAS score at rest and cough 24 hours after operation and those indexes at 48 hours after operation. The VAS scores at rest and cough were analyzed by Revman 5.2 software. Results seven randomized controlled trials were included, and a total of 362 patients were included.Compared with the thoracic paravertebral nerve block group, There were no significant differences in the vertical spinal plane block group in the VAS scores at 24 hours and VAS scores at 48 hours after operation when patients were at rest or cough(P>0.05). Conclusion There is no significant difference in postoperative VAS score between ultrasound-guided paravertebral nerve block and vertical spinal muscle plane nerve block in patients undergoing thoracic surgery.
