摘  要:目的 探讨同伴教育对减重术后病人心理、饮食依从性和生活质量的影响。方法 选取2018年1月至2020年12月在南京医科大学第一附属医院减重代谢外科实施减重手术的124例病人,按随机数字表法分成对照组(61例)和观察组(63例),对照组实施常规教育,观察组采用同伴教育,干预时间为出院后至术后6个月。分别采用焦虑自评量表(SAS)、抑郁自评量表(SDS)、自制饮食依从性量表、SF-36健康调查量表,评估病人心理、术后饮食依从性和生活质量。结果 两组病人焦虑自评量表和抑郁自评量表的得分均较干预前下降,且观察组低于对照组,差异有统计学意义(P<0.05);饮食依从性观察组病人显著优于对照组,差异有统计学意义(P<0.05)。两组病人在生理健康各维度得分均较干预前提高,差异有统计学意义(P<0.05)。观察组病人经同伴教育干预后,心理健康中的活力、社会功能和精神健康这三个维度的得分明显优于对照组(P<0.05)。结论 实施同伴教育有利于改善减重术后病人焦虑、抑郁的负性心理,提高饮食依从性,提升生活质量。Objective To explore the effects of peer education on psychology, dietary compliance and quality-of-life in patients after bariatric surgery.Methods From January 2018 to December 2020, 124 patients undergoing bariatric surgery were recruited and randomized into two groups of control(n=61) and observation(n=63). Control group received routine education while observation group peer education. Intervention period started from hospital discharge to 6 months post-operation. Self-rating anxiety scale(SAS), self-rating depression scale(SDS), self-made dietary compliance scale and SF-36 health questionnaire were utilized for evaluating patient psychology, postoperative dietary compliance and quality-of-life.Results The scores of SAS/SDS declined in both groups after intervention and observation group was lower than control group(P<0.05). Dietary compliance of observation group was significantly better than that of control group(P<0.05). The scores of physical health improved in both groups as compared with those pre-intervention(P<0.05). In terms of mental health, the scores of vitality, social function and mental health were significantly higher in observation group than those in control group after peer education(P<0.05).Conclusion Peer education may facilitate reducing negative psychology of anxiety and depression, enhancing dietary compliance and boosting quality-of-life after bariatric surgery.
