2型糖尿病并发脑梗死患者应用芪棱汤联合西医方案治疗的效果  

Effect of Qiling Decoction Combined with Western Medicine Programme Treatment on Patients with Type 2 Diabetes Complicated with Cerebral Infarction

摘　　要：目的:探讨2型糖尿病(T2DM)并发脑梗死患者应用芪棱汤联合西医方案治疗的效果。方法:选取麻城市中医医院2020年2月-2021年7月收治的T2DM并发脑梗死患者124例。按随机数字表法将其分为观察组(62例)和对照组(62例)。对照组采用西医方案治疗,观察组在此基础上联合芪棱汤进行治疗。比较两组临床疗效,治疗前后氧化应激指标、炎症因子、血管内皮生长因子(VEGF)、血小板衍化内皮细胞生长因子(PD-ECGF)。结果:观察组总有效率明显比对照组更高(P<0.05)。治疗后,观察组血清超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-Px)水平均高于对照组(P<0.05),血清丙二醛(MDA)水平低于对照组(P<0.05)。两组治疗后血清肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)和白细胞介素-6(IL-6)水平均降低,且观察组血清IL-6、TNF-α、IL-1β水平均低于对照组(P<0.05)。两组治疗后血清PD-ECGF、VEGF水平均降低,且观察组血清PD-ECGF、VEGF水平均低于对照组(P<0.05)。结论:芪棱汤联合西医方案治疗可以有效改善T2DM患者并发脑梗死患者的神经功能缺损程度,抑制机体炎症反应,改善抗氧化能力和机体的内皮细胞功能,疗效确切。Objective:To investigate the effect of Qiling Decoction combined with western medicine programme treatment on patients with type 2 diabetes mellitus(T2DM)complicated with cerebral infarction.Method:A total of 124 patients with T2DM complicated with cerebral infarction treated in Macheng Traditional Chinese Medicine Hospital from February 2020 to July 2021 were selected.They were divided into observation group(62 cases)and control group(62 cases)according to random number table method.The control group was treated with western medicine programme,and the observation group was treated with Qiling Decoction on this basis.The clinical effects,oxidative stress index,inflammatory factor,vascular endothelial growth factor(VEGF)and platelet-derived endothelial cell growth factor(PD-ECGF)before and after treatment of the two groups were compared.Result:The total effective rate of the observation group was significantly higher than that of the control group(P<0.05).After treatment,the levels of serum superoxide dismutase(SOD),glutathione peroxidase(GSH-Px)in the observation group were higher than those in the control group(P<0.05),and the level of serum malondialdehyde(MDA)was lower than that in the control group(P<0.05).After treatment,the levels of serum tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β)and interleukin-6(IL-6)in the two groups were decreased,and the levels of serum IL-6,TNF-αand IL-1βin observation group were lower than those in control group(P<0.05).After treatment,the levels of serum PD-ECGF and VEGF of the two groups were decreased,and those in the observation group were lower than those in the control group(P<0.05).Conclusion:Qiling Decoction combined with western medicine programme treatment can effectively improve the degree of neurological function deficit in patients with T2DM complicated with cerebral infarction,inhibit the inflammatory response of body,and improve the antioxidant capacity and endothelial cell function,with definite curative effect.
