益肾降浊汤治疗脓毒症急性肾损伤患者的临床效果

摘　　要：目的:观察益肾降浊汤治疗脓毒症急性肾损伤患者的临床效果。方法:选取2021年3-9月福建中医药大学附属人民医院急诊科收治的60例脓毒症急性肾损伤患者,按随机数字表法分为两组,各30例。对照组采用西医常规治疗,观察组在对照组的基础上加用益肾降浊汤,比较治疗前后两组序贯性器官功能衰竭评估(SOFA)评分、中医临床症候积分、尿量、血肌酐、尿中性粒细胞明胶酶相关脂质运载蛋白(NGAL)、尿肾损伤分子-1(KIM-1)。结果:治疗前,两组SOFA评分、中医临床症候积分、尿量、血肌酐、尿NGAL、尿KIM-1比较差异均无统计学意义(P>0.05);治疗后,两组SOFA评分和中医临床症候评分均低于治疗前(P<0.05),且观察组SOFA评分和中医症候评分明显低于对照组(P<0.05);治疗后,两组尿量较治疗前增多(P<0.05),且观察组尿量明显多于对照组(P<0.05);治疗后,两组血肌酐、尿NGAL和尿KIM-1水平低于治疗前(P<0.05),且观察组血肌酐、尿NGAL和尿KIM-1水平明显低于对照组(P<0.05)。结论:益肾降浊汤能改善脓毒症急性肾损伤患者的肾功能,缓解临床症状。Objective:To observe the clinical efficacy of Yishen Jiangzhuo Soup in the treatment of patients with sepsis acute kidney injury.Method:Sixty patients with sepsis acute kidney injury admitted to the Emergency Department of Affiliated People’s Hospital of Fujian University of Chinese Medicine from March to September 2021 were selected and divided into two groups according to random number table method of 30 cases each.The control group was treated with conventional western medicine,and the observation group was treated with Yishen Jiangzhuo Soup on the basis of the control group.The sequential organ failure assessment(SOFA)score,TCM clinical symptom score,urine volume,blood creatinine,urine neutrophil gelatinase-associated lipocalin(NGAL),urine kidney injury molecule-1(KIM-1)in both groups before and after treatment were compared.Result:Before treatment,there were no statistically significant differences in SOFA score,TCM clinical symptom score,urine volume,blood creatinine,urine NGAL,and urine KIM-1 between the two groups(P>0.05).After treatment,SOFA score and TCM clinical syndrome score in both groups were lower than before treatment(P<0.05),and SOFA score and TCM clinical syndrome score in the observation group were significantly lower than those in the control group(P<0.05);after treatment,urine volume increased in both groups compared with that before treatment(P<0.05),and the urine volume of observation group was significantly higher than that of control group(P<0.05);after treatment,the levels of serum creatinine,urine NGAL and urine KIM-1 in both groups were lower than before treatment(P<0.05),and the serum creatinine,urine NGAL and urine KIM-1 levels in the observation group were significantly lower than those in the control group(P<0.05).Conclusion:Yishen Jiangzhuo Soup can improve renal function and relieve clinical symptoms in patients with sepsis acute kidney injury.
