股骨近端防旋髓内钉与动力髋螺钉治疗股骨粗隆间骨折的临床效果对比

摘　　要：目的:探究对比股骨近端防旋髓内钉与动力髋螺钉治疗股骨粗隆间骨折的临床效果。方法:回顾性分析江门市人民医院从2019年4月-2021年5月收治120例的股骨粗隆间骨折患者,按抽签法分为对照组和研究组,每组60例。对照组进行动力髋螺钉干预,研究组给予股骨近端防旋髓内钉干预。比较两组患者的手术及术后康复指标情况、髋关节功能评分(Harris)、髋关节伸屈活动度(ROM)、视觉模拟评分(VAS)情况、术后并发症发生情况。结果:研究组的切口长度、手术时间、骨折愈合时间、术中出血量及出院时间指标均优于对照组(P<0.05);干预后,两组Harris评分、ROM高于干预前,VAS评分低于干预前(P<0.05),且研究组Harris评分、ROM高于对照组,VAS评分低于对照组(P<0.05);术后,研究组并发症发生率为6.67%,低于对照组的20.00%,差异有统计学意义(χ^(2)=4.615,P<0.05)。结论:与动力髋螺钉对比,股骨近端防旋髓内钉治疗股骨粗隆间骨折患者具有改善手术及术后康复指标,改善髋关节功能、伸屈活动度、缓解疼痛,且并发症较少等优势。Objective:To explore and compare the clinical effects of proximal femoral anti rotation intramedullary nail and dynamic hip screw in the treatment of femoral intertrochanteric fracture.Method:A retrospective analysis of 120 patients with femoral intertrochanteric fractures who were admitted to the People’s Hospital of Jiangmen from April 2019 to May 2021 were divided into control group and study group by lottery,with 60 patients in each group.The control group was treated with dynamic hip screw,and the study group was treated with proximal femoral anti rotation intramedullary nail.The operation and postoperative rehabilitation indexes,hip function scores(Harris),hip extension and flexion range of motion(ROM),visual analog scores(VAS)and postoperative complications were compared between the two groups.Result:The incision length,operation time,fracture healing time,intraoperative bleeding and discharge time in the study group were better than those in the control group(P<0.05);after the intervention,the Harris scores and ROM of the two groups were higher than those before the intervention,and the VAS score was lower than that before the intervention(P<0.05).The Harris score and ROM of the study group were higher than those of the control group,and the VAS score was lower than that of the control group(P<0.05);after operation,the incidence of complications in the study group was 6.67%,lower than 20.00% in the control group,the difference was statistically significant(χ^(2)=4.615,P<0.05).Conclusion:Compared with dynamic hip screw,proximal femoral anti rotation intramedullary nail in the treatment of femoral intertrochanteric fracture has the advantages of improving surgical and postoperative rehabilitation indexes,improving hip function,extension and flexion range of motion,relieving pain,and less complications.
