摘 要:目的:探讨并分析超声联合CT引导定位双极脉冲射频缓解神经根型颈椎病患者疼痛的作用,以有效提高颈椎病的治疗效果。方法:选择医院诊治的126例神经根型颈椎病患者,采用随机数表法将其分为观察组和对照组,每组63例。观察组采用彩色超声仪,22 G射频针给予双极脉冲射频治疗;对照组给予硬膜外注射镇痛治疗。对比两组治疗总有效率,肌电图颈神经F波传导速度,以及并发症发生情况;采用疼痛数字评级量表(NRS)评分,分析两组患者的疼痛变化情况。结果:两组患者治疗过程中均无严重并发症发生;观察组治疗总有效率为98.4%,显著高于对照组(88.9%),差异有统计学意义(x^(2)=4.805,P<0.05)。观察组治疗第1周和第4周的NRS评分均显著低于对照组,差异有统计学意义(t=8.822,t=6.922;P<0.05)。观察组和对照组治疗后患者的肌电图F波传导速度均显著高于治疗前,差异有统计学意义(t=11.945,t=6.292;P<0.05);治疗后观察组患者颈神经F波传导速度显著高于对照组,差异有统计学意义(t=5.333,P<0.05)。结论:超声联合CT双极脉冲射频能持续促进缓解神经根型颈椎病患者的疼痛状况,加快肌电图F波传导速度,从而提高治疗效果。Objective: To investigate and analysis the therapeutic effects of bipolar pulse radiofrequency in relieving pain of patients with nerve-root type cervical spondylosis. Methods: 126 patients with nerve-root type cervical spondylosis who admitted to hospital were selected and they were equally divided into the observation group and the control group accorded to the random number table method, with 63 cases in each group. The observation group adopted color ultrasound instrument and underwent the treatment of bipolar pulse radiofrequency with 22G radiofrequency needle, and control group underwent epidural injection for analgesia therapy. The total effective rate of treatment, conduction velocity of F wave of electromyography and the occurrence of complication between two groups were compared, and numerical rating scale(NRS) was adopted to analyze the pain change between two groups. Results: There were no serious complications in the process of treatment in the two groups. The total effective rate of treatment in observation group was 98.4%, which was significantly higher than 88.9% of control group(x^(2)=4.805, P<0.05). The NRS scores at the first and fourth weeks after treatment in observation group were significantly lower than those in control group(t=8.822, t=6.922, P<0.05), respectively. The conduction velocities of F wave of electromyography of two groups after treatment were significantly higher than those before treatment(t=11.945, t=6.292, P<0.05), respectively. And the conduction velocity of F wave of electromyography after treatment in observation group was significantly higher than that in control group(t=5.333, P<0.05). Conclusion: Bipolar pulse radiofrequency can continuously promote the relief of pain in patients with nerve-root type cervical spondylosis, and accelerate the conduction velocity of F wave of electromyography, thus improve the therapeutic effect.
