摘 要:目的 了解南京地区HIV-1 CRF01_AE毒株分子流行特征及传播网络情况,为该毒株的疫情防控提供参考依据。方法 收集2015年9月至2019年6月南京市新确诊的HIV-1感染者血样及人口学信息。扩增HIV-1 pol区基因片段,经判定CRF01_AE毒株共401条。以最大似然法构建系统进化树;通过Tamura-Nei 93成对基因距离法构建传播网络并分析其特征。结果 CRF01_AE毒株在南京地区共形成了3个全国相关流行枝(Clade 4a、Clade 4b、Clade 5)以及1个独特的南京流行枝(NJ-Clade),这4个流行枝分别包括23.69%(95/401)、18.20%(73/401)、13.72%(55/401)和30.92%(124/401)的南京序列。NJ-Clade中感染者年龄显著低于其他流行枝(χ^(2)=34.286,P<0.001),并且具有更高比例的同性性向(χ^(2)=19.129,P=0.024)和男男性行为(χ^(2)=14.293,P=0.027)。71.82%(288/401)的CRF01_AE毒株形成83个传播网络。多因素分析显示,相比Clade 4a(a OR=0.464)、Clade 4b(a OR=0.363)、Clade 5(a OR=0.483)流行枝,NJ-Clade具有更高的网络形成率。结论 南京地区HIV-1 CRF01_AE毒株流行情况复杂,除包含全国相关流行枝外,已形成了独特的南京流行枝,应持续监测各流行枝的进化传播情况。Objective To understand the molecular epidemiological characteristics and transmission network of the HIV-1 CRF01_AE strain in Nanjing and provide a reference for the prevention and control of this strain.Methods We collected blood samples and demographic information of newly infected people with HIV-1 in Nanjing from September2015 to June 2019 and amplified the HIV-1 pol gene fragment.A total of 401 CRF01_AE strains were determined.The phylogenetic tree was constructed by the maximum likelihood method.The propagation network was constructed by the Tamura-Nei 93 paired gene distance method,and its characteristics were analyzed.Results The CRF01_AE strain has formed three epidemic branches related to nationwide in Nanjing,including Clade 4a,Clade 4b,Clade 5,and 1 unique Nanjing epidemic branch(NJ-Clade).These 4 epidemic branches included 23.69%(95/401),18.20%(73/401),13.72%(55/401) and 30.92%(124/401) sequences respectively.The age of infected persons in NJ-Clade was significantly lower than that of other epidemics(χ^(2)=34.286,P<0.001),and they had a higher proportion of homosexual orientation(χ^(2)=19.129,P=0.024) and the behavior of men who have sex with men(χ^(2)=14.293,P=0.027).71.82%(288/401) of CRF01_AE strains formed 83 transmission networks.Multi-factor analysis shows that NJ-Clade has a higher network formation rate compared to the popular branches of Clade 4a(a OR=0.464),Clade 4b(a OR=0.363),and Clade 5(a OR=0.483).Conclusions Due to the complicated epidemic situation of the HIV-1 CRF01_AE strain in the Nanjing area,a unique Nanjing epidemic branch has been formed in addition to the relevant epidemic branches in the country.We suggest that the evolutionary spread of each epidemic branch should be continuously monitored.
