摘  要:目的 了解HIV/AIDS患者接种新型冠状病毒(简称新冠病毒)灭活疫苗后抗体反应和不良反应发生情况,探讨影响抗体反应的因素。方法 招募接种两剂新冠病毒灭活疫苗的HIV/AIDS患者,问卷收集不良反应信息和采集血液检测特异性IgG、IgM抗体;使用“艾滋病防治基本信息系统”收集社会人口学信息和随访信息;对HIV/AIDS患者接种第二剂疫苗后21~35天特异性IgG抗体阴性的影响因素进行单因素和多因素非条件Logistic回归分析。结果 共招募HIV/AIDS患者177名,平均年龄(34.17±8.25)岁;共有148份血样特异性IgG抗体检测呈阳性,抗体阳转率为83.62%,IgG抗体阳性S/CO值的均值为(9.67±10.53),单因素分析显示第一剂新冠病毒疫苗接种前的ART时间、病毒载量和CD4细胞计数与IgG抗体阴性相关。多因素Logistic回归结果显示:第一剂疫苗接种前CD4细胞计数高于200个/μL的HIV/AIDS患者,IgG抗体呈阴性的概率越低。问卷结果显示:注射部位疼痛和嗜睡分别是最常见的局部、全身不良反应,不良反应一般无需特殊治疗,自行缓解。结论 HIV/AIDS患者接种新冠病毒灭活疫苗后,具有较好的免疫应答率和安全性,CD4细胞计数超过200个/μL时接种应答率更高。Objective To understand both antibody and adverse reactions in HIV-infected individuals after vaccination with inactivated SARS-CoV-2 vaccine and to explore the factors affecting antibody reactions.Methods We recruited HIV/AIDS patients vaccinated with a two-dose inactivated SARS-CoV-2 vaccine.Questionnaires collected information on adverse reactions and blood tests for specific IgG and IgM antibodies.In addition,collect the social demographic information and follow-up information from the AIDS prevention and control information system.The influencing factors of specific IgG antibody negative in HIV infected individuals 21-35 days after the second dose of vaccine were analyzed by univariate and multivariate unconditional logistic regression.Results 177 HIV-infected individuals were recruited,with an average age of(34.17±8.25) years.148 blood samples were positive for specific IgG antibodies,with an antibody-positive conversion rate of 83.62% and the mean S/CO value of IgG antibody positive as(9.67±10.53).ART duration,viral load,and CD4 cell count before the first dose of inactivated SARS-CoV-2 vaccine were statistically correlated with negative IgG antibodies.The results of multivariate Logistic regression analysis showed that the probability of IgG antibody negative was lower in HIV-infected individuals whose CD4 cell count was higher than 200/μL before the first vaccine vaccination.The questionnaire results showed that pain at the injection site and drowsiness were the most common local and systemic adverse reactions.Generally,the adverse reactions were relieved by themselves without special treatment.Conclusions HIV-infected individuals have an excellent immune response rate and safety after being vaccinated with inactivated SARS-CoV-2 vaccine.The vaccination response rate is higher when the CD4 cell count exceeds 200/μL.
