不同培养基的淋球菌培养性能及药物敏感性检测适用性评估

摘　　要：目的对多种培养基的淋球菌培养性能及用于纸片扩散法药物敏感性检测的准确性进行评估。方法将4株淋球菌标准菌株及20株临床分离株分别接种于不同培养基上,观察评估菌株的生长状况;随后选取生长状况较好的培养基利用纸片扩散法测定菌株对头孢曲松和头孢克肟的药物敏感性,与琼脂稀释法结果进行比较,计算各培养基检测结果的药敏程度(耐药/中敏/敏感)符合率与误差率。结果淋球菌在各培养基上生长状况不同,4种哥伦比亚培养基上生长状况范围在1.25~2.21,较其他培养基好,4种Mueller-Hinton培养基上生长状况为0.00~0.04,不适合用于淋球菌培养。实验室自配GC羊血琼脂培养基在两种药物检测中的符合率均为100.00%;商用培养基中,科马嘉巧克力琼脂培养基(不加抗生素)和安图哥伦比亚血琼脂培养基分别在头孢曲松和头孢克肟耐药性检测中较其他种类具有最高的药敏符合率,分别为100.00%和95.83%。结论采用纸片扩散法对淋球菌药物敏感性进行定性检测时,优先使用自行配置的GC琼脂羊血培养基,如需使用商业培养基,推荐使用药敏符合率高、误差率低的商用培养基。Objective To evaluate the culture performance for Neisseria gonorrhoeae and the accuracy of gonococcal antimicrobial susceptibility test by disk diffusion method in various media.Methods 4 standard strains and20 clinical isolates of Neisseria gonorrhoeae were inoculated into each culture medium to observe the growth status.The medium providing good growth status was selected to identify the drug resistance of ceftriaxone and cefixime through the disk diffusion method.Ultimately the results were compared with the agar dilution method.Results The growth status of Neisseria gonorrhoeae on each culture medium was inconsistent.The growth status on the four Columbia media ranged from 1.25 to 2.21,which was better than the other media,and 0.00 to 0.04 on the four Mueller-Hinton media,showing the inapplicability for gonococcal culture.Moreover,the category agreement of sheep blood-containing GC agar medium in detecting the susceptibility of two antibiotics was 100%.Among the commercial media,Chromagar chocolate agar medium(without antibiotics) and Autobio Columbia blood agar medium had the highest category agreement in ceftriaxone and cefixime susceptibility testing,100.00% 95.83%,respectively.Conclusions In the qualitative detection of gonococcal drug resistance by the disk diffusion method,the self-prepared sheep blood-containing GC agar medium is recommended.If the commercial medium is required,the one with a higher category agreement is recommended for detection.
