妊娠期糖尿病患者血清半胱氨酸、脂肪因子与糖脂代谢及炎症反应的相关性分析

摘　　要：目的探讨妊娠期糖尿病患者血清半胱氨酸、脂肪因子与糖脂代谢及炎症反应的相关性。方法选取50例妊娠期糖尿病患者为病例组,同期健康体检的50例正常孕妇作为对照组。检测两组血清半胱氨酸(Hcy)、脂肪因子趋化素和瘦素水平;检测两组胰岛功能、糖脂代谢标志物及炎症因子水平。结果病例组血清Hcy、趋化素及瘦素水平显著高于对照组;胰岛功能标志物空腹胰岛素(FINS)、餐后2 h胰岛素(2hINS)、胰岛素抵抗指数(HOMA-IR)显著高于对照组;糖脂代谢标志物糖化血红蛋白(HbAlc)、空腹血糖(FPG)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDLC)水平均显著高于对照组,高密度脂蛋白胆固醇(HDLC)水平低于对照组(P<0.05)。Pearson相关性分析结果显示,病例组血清Hcy、趋化素、瘦素与FINS、2hINS、HOMA-IR、HbAlc、FPG、TC、TG、LDLC、CRP、IL-6、TNF-α水平呈正相关(P<0.05),与HDLC水平呈负相关(P<0.05)。结论妊娠期糖尿病患者血清Hcy和脂肪因子与糖脂代谢紊乱以及慢性炎症反应密切相关,可能参与了妊娠期糖尿病的发生与发展。Aim To investigate the correlation of serum cysteine adipokine with glucose and lipid metabolism and inflammatory response in gestational diabetes mellitus.Methods Fifty patients with gestational diabetes were selected as the case group,and 50 healthy pregnant women who underwent physical examination at the same period were selected as the control group.The indexes including islet function,glycolipid metabolism markers and inflammatory factors were detected.Results The serum levels of Hcy,chemokine and leptin in case group were significantly higher than those in control group.The islet functional markers,fasting insulin(FINS),2 h postprandial insulin(2 hINS),insulin resistance index(HOMA-IR)in case group were significantly higher than those in control group.The levels of glucolipid metabolism indexes glycated hemoglobin(HbAlc),fasting blood glucose(FPG),total cholesterol(TC),triglyceride(TG)and low density lipoprotein cholesterol(LDLC)were significantly higher than those in control group,while the level of high density lipoprotein cholesterol(HDLC)was lower than that in control group(P<0.05).Pearson correlation analysis showed that serum Hcy,chemokine and leptin were positively correlated with FINS,2 hINS,HOMA-IR,HbAlc,FPG,TC,TG,LDLC,CRP,IL-6 and TNF-αlevels(P<0.05)and negatively correlated with HDLC level(P<0.05).Conclusion Serum Hcy and adipokine in patients with gestational diabetes are closely related to the disorder of glucose and lipid metabolism and chronic inflammatory response,which may be involved in the occurrence and development of gestational diabetes.
