摘 要:目的 分析红景天对蛛网膜下腔出血(SAH)所致脑血管痉挛的防治效果及可能机制。方法 180例SAH患者入院后随机均分为对照组和红景天组,两组常规对症治疗,红景天组加用大株红景天注射液治疗。比较两组脑实质循环和脑血管痉挛情况、凝血指标、血管内皮活性、氧化应激和炎症因子水平。结果 与治疗前比较,治疗后两组脑循环和末端周围循环时间均下降,大脑中动脉血管直径上升(P<0.05);治疗后两组一氧化氮、抗凝血酶Ⅲ、超氧化物歧化酶水平上升,内皮素、纤溶酶原活化素抑制剂、组织型纤溶酶原活化剂、血栓素、丙二醛、肿瘤坏死因子-α和白细胞介素-6水平下降(P<0.05);且红景天组较对照组更为显著(P<0.05)。红景天组脑血管痉挛发生率低于对照组(P<0.05)。结论 红景天可改善脑微循环,促进血管舒张,抑制血栓形成,降低氧化应激和炎症反应,对SAH后脑血管痉挛有较好疗效。Aim To analyze the control effect of rhodiola on cerebral vascular spasm induced by subarachnoid hemorrhage(SAH), and its possible mechanism. Methods 180 patients with SAH were randomly and equally divided into control group and rhodiola group after admission. Both groups were given conventional symptomatic treatment, and the rhodiola group was additionally treated with rhodiola wallichiana injection. The status of cerebral parenchymacirculation and cerebral vascular spasm, the levels of coagulation indicators, vascular endothelial activity, oxidative stress and inflammatory factors were compared between the two groups. Results After treatment, cerebral circulation time and terminal peripheral circulation were reduced, and the diameter of middle cerebral artery was increased in the two groups(P<0.05). After treatment, the levels of nitric oxide, antithrombin III and superoxide dismutase were increased, while the levels of endothelin, plasminogen activator inhibitor, tissue type plasminogen activator, thromboxane, malondialdehyde, tumor necrosis factor α and interleukin-6 were reduced in the two groups(P<0.05). Meanwhile, changes of these indicators were more significant in rhodiola group than in control group(P<0.05). The incidence of cerebral vascular spasmin rhodiola group was lower than that in control group(P<0.05). Conclusions rhodiola can improve cerebral microcirculation, promote vasodilation, inhibit thrombosis and reduce oxidative stress and inflammatory response. It is effective in the treatment of cerebral vascular spasm induced by SAH.
