摘  要:朱镇华认为,耳鸣的发病不可忽视脾虚病机,其常以脾胃气虚为本。气虚则气血生化无源,耳窍失养,发为耳鸣;气虚易生痰湿,久痰生内瘀,痰瘀互结,阻碍气机,清阳不升,耳窍不通利,亦可见耳鸣。治疗上,朱镇华以健脾益气、渗湿化瘀通窍为治法,方用参苓白术散加山萸肉、石菖蒲、桃仁、红花和地龙等。通过对参苓白术散加味治疗脾虚痰瘀型耳鸣的中医理论进行探讨,以期更好地指导临床用药。ZHU Zhen-hua believes that spleen deficiency is the important factor of tinnitus and that this factor of tinnitus should not be ignored.Spleen deficiency leads to the biochemical absence of Qi and blood easily,as well as loss of ear cultivation for tinnitus.QI deficiency is easy to cause phlegm and dampness.Phlegm is easy to produce blood stasis after a long time.Phlegm and blood stasis mutually knot,block Qi.Qingyang(清阳)does not rise,which leads to earing obstruction.And it can also lead to tinnitus.On the treatment,Professor ZHU Zhen-hua applies the method of invigorating spleen Qi and infiltration of dampness to remove blood stasis and restore hearing of the ear.And the prescription with Shenling Baizhu San supplemented by Shanyurou(Corni Fructus),Shichanpu(Acori Tatarinowii Rhizoma),Taoren(Peach Kernel),Honghua(Safflower)and Dilong(Earthworm).This study conducted a certain extent of the Chinese medicine theory of Shenling Baizhu San to treat tinnitus of spleen deficiency and sputum stasis syndrome,in order to better guide clinical medication.
