《吴鞠通医案》温病误治纠误探析

摘　　要：通过梳理《吴鞠通医案》中前医诊疗温病的误治案例,如误用辛温、养阴、寒凉及攻下等法而导致变证的病机及吴鞠通相应的纠误思想,总结吴鞠通辨治温病特色。温病误治所导致的变证病机复杂,变化多端。热邪为温病之根本,阴伤为温病之标,病变由热邪导致。治疗温病误治变证当首先明晰温热邪气所在具体部位,以祛除热邪为治疗的首要目标,热除则正安阴复;阴液损伤为温热邪气所致,是温病的主要变证,阴伤重时应急救阴液,扶正祛邪;温病误治易导致气机不通,可采用宣通气机之法治疗气机不通之变证;温病误治易损伤脾胃阴液和阳气,要注意顾护脾胃阴液与脾胃运化功能。This paper summarizes the characteristics of Wu's diagnosis and treatment of febrile diseases according to sorting out the cases of mistreatment of febrile diseases by former doctors in Wu Ju Tong Medical Case,such as the pathogenesis of the syndrome variation caused by the mistaken use of pungent-warm,nourishing Yin,cold and offensive purgative Chinese medicine,as well as WU Ju-tong's corresponding ideas of error correction.The syndrome variation caused by the mistreatment is complex and variable.Among them,hot evil is the root of febrile disease,and Yin injury is the mark of warm disease.The lesion is caused by hot evil.Therefore,to treat febrile disease,it is necessary to remove heat evil as the primary goal.Heat evil removal leads to healthy Qi and Yin recovery.Yin fluid damage is caused by warm heat and evil Qi,which is the main syndrome variation of febrile disease.The treatment for Yin injury should be aiding the Yin fluid and strengthening the body resistance to eliminate pathogenic factors.Febrile disease is easy to cause Qi obstruction,so it can be treated by clearing and Dredging.Mistreatment of febrile diseases is easy to damage Yin fluid and Yang Qi of spleen and stomach,so attention should be paid to strengthening spleen stomach Yin fluid and transportation function of spleen stomach.
