摘　　要：目的探讨头皮扩张皮瓣联合三维钛网治疗颅骨修补术后钛网外露的效果。方法自2016年1月至2019年8月,空军军医大学第一附属医院整形外科共收治7例钛网外露伴严重感染患者,外露面积为1.0 cm×0.5 cm~2.5 cm×2.5 cm。手术分为三期:一期去除钛网并彻底清创,至少6个月后,二期在颅骨缺损旁埋置长方形扩张器,待头皮充分扩张后,三期行三维钛网修复颅骨缺损,扩张头皮旋转修复创面。结果7例患者术后获随访24~36个月,扩张皮瓣均完全成活,无钛网外露。患者对术后外观满意。结论采用头皮扩张皮瓣联合三维钛网,是修复钛网外露伴严重感染的有效方法。Objective To explore the efficacy of expanded scalp flap combined with 3D titanium mesh in the repair of titanium mesh exposure after cranioplasty.Methods From January 2016 to August 2019,7 patients with titanium mesh exposure and severe infection were collected.The exposed area ranged from 1.0 cm×0.5 cm to 2.5 cm×2.5 cm.Surgical procedures were divided into 3 stages.In the first stage,the titanium mesh was removed and thoroughly debridement was performed.At least 6 months later,a rectangle expander was embedded next to the cranial defect and scalp soft tissue expansion was performed in the second stage.In the third stage,the cranial defect was repaired by 3D titanium mesh and the wounds were repaired by expanded scalp flap.Results Follow-up ranged from 24 months to 36 months,all flaps survived without titanium mesh exposure.Patients were satisfied with postoperative appearance.Conclusions The expanded scalp flap combined with 3D titanium mesh is an effective way to repair titanium mesh exposure with severe infection after cranioplasty.
