心可舒片联合降压治疗对原发性高血压合并焦虑抑郁患者血压和血压变异性及焦虑抑郁程度的影响

摘　　要：目的探讨心可舒片联合降压治疗对原发性高血压合并焦虑抑郁患者的血压、血压变异性(BPV)及焦虑抑郁程度的影响。方法选取2020年1月至2021年1月在济宁医学院附属滕州市中心人民医院心内科门诊诊断原发性高血压2级伴有焦虑抑郁患者85例。按随机数字表法分为观察组(44例)和对照组(41例)。对照组进行生活方式干预及降压药物治疗;观察组在对照组基础上加用心可舒片治疗,4片/次,3次/d口服,持续4周。比较2组服药前后血压、BPV和焦虑自评量表(SAS)、抑郁自评量表(SDS)评分以及不良反应发生情况。结果服药4周后2组日间收缩压、日间舒张压、夜间收缩压、夜间舒张压均明显低于服药前[对照组:(123±27)mmHg(1 mmHg=0.133 kPa)比(151±37)mmHg、(81±14)mmHg比(92±22)mmHg、(112±22)mmHg比(131±29)mmHg、(65±18)mmHg比(78±15)mmHg;观察组:(122±24)mmHg比(153±35)mmHg、(80±16)mmHg比(94±30)mmHg、(110±23)mmHg比(129±30)mmHg、(64±20)mmHg比(79±22)mmHg](均P<0.05)。服药前后2组日间收缩压、日间舒张压、夜间收缩压、夜间舒张压组间比较差异均无统计学意义(均P>0.05)。服药4周后,2组日间收缩压标准差、日间舒张压标准差、夜间收缩压标准差(nSBPSD)、夜间舒张压标准差均低于服药前(均P<0.05)。服药前2组间BPV各指标比较差异均无统计学意义(均P>0.05);服药4周后观察组nSBPSD明显低于对照组(P<0.01)。服药4周后,观察组SAS、SDS评分低于服药前和对照组[(48±4)分比(58±10)、(53±6)分;(49±4)分比(55±8)、(53±5)分],差异均有统计学意义(均P<0.05)。对照组服药前后SAS、SDS评分比较差异均无统计学意义(均P>0.05)。治疗过程中,2组患者均无明显不良反应发生。结论心可舒片联合降压治疗能明显降低原发性高血压合并焦虑抑郁患者血压水平,改善BPV,缓解患者焦虑抑郁情绪,并且无明显不良反应。Objective To explore the effects of Xinkeshu tablets combined with antihypertensive therapy on blood pressure,blood pressure variability(BPV)and the degree of anxiety and depression in patients with hypertension complicated with anxiety and depression.Methods From January 2020 to January 2021,85 patients with grade 2 essential hypertension complicated with anxiety and depression diagnosed in Department of Cardiology Clinic,Tengzhou Central People′s Hospital Affiliated to Jining Medical University were selected.They were randomly divided into observation group(44 cases)and control group(41 cases).The control group was treated with lifestyle intervention and antihypertensive drugs;the observation group was treated with Xinkeshu tablets on the basis of the control group,4 tablets/time,3 times/d orally for 4 weeks.The blood pressure,level of BPV and scores of self-rating anxiety scale(SAS)and self-rating depression scale(SDS)were compared between the two groups before and after treatment,and the occurrence of adverse reactions was observed.Results After 4 weeks of medication,the day systolic blood pressure,day diastolic blood pressure,night systolic blood pressure,and night diastolic blood pressure of two groups were significantly lower than those before treatment[control group:(123±27)mmHg vs(151±37)mmHg,(81±14)mmHg vs(92±22)mmHg,(112±22)mmHg vs(131±29)mmHg,(65±18)mmHg vs(78±15)mmHg;observation group:(122±24)mmHg vs(153±35)mmHg,(80±16)mmHg vs(94±30)mmHg,(110±23)mmHg vs(129±30)mmHg,(64±20)mmHg vs(79±22)mmHg](all P<0.05).There were no significant differences in dSBP,dDBP,nSBP and nDBP between the two groups before and after treatment(all P>0.05).After 4 weeks of treatment,the day systolic blood pressure standard deviation,day diastolic blood pressure standard deviation,night systolic blood pressure standard deviation(nSBPSD),and night diastolic blood pressure standard deviation of two groups were significantly lower than those before treatment(all P<0.05).There were no significant differences in BPV
