基于Tabu搜索算法的喜炎平注射液治疗疱疹性咽峡炎的真实世界药物复杂网络分析

摘　　要：目的针对真实世界中喜炎平注射液治疗疱疹性咽峡炎及合并症住院人群联用药物情况,应用Tabu搜索算法进行群组模块分析,筛选出喜炎平注射液治疗疱疹性咽峡炎及合并症临床药物用药方案。方法基于Tabu搜索算法,分析全国39家三甲医院信息系统(HIS)数据库中使用喜炎平注射液治疗疱疹性咽峡炎3157例患者的药物复杂网络,筛选中西医联合用药方案。结果无神经系统受累者用药考虑:喜炎平联合维生素类、非甾体抗炎药、黏膜外用药;神经系统受累者用药考虑:喜炎平联合甘露醇、醒脑静、糖皮质激素;合并肺部感染者用药考虑:喜炎平联合抗生素、清热解毒剂、化痰平喘药。结论该研究分析了真实世界中喜炎平注射液治疗疱疹性咽峡炎及合并症住院人群联用药物情况,针对不同病情分层提出了喜炎平注射液联合药物治疗方案,其与本病临床特征、病情变化及相应的指南/共识相符,可供临床医师参考。Objective In this study,using Tabu search algorithm for group module analysis to explore Xiyanping Injection for the treatment of herpangina and the combined use of drugs in hospitalized patients with comorbidities in the real-world,we analyzed and screened the optimal plan for clinical medicine and combination with Chinese and Western medicine of Xiyanping Injection for the treatment of herpangina and its complications.Methods Based on the Tabu search algorithm,we analyzed the complex network of Xiyanping Injection in treating 3157 patients with herpangina in the information system(HIS)database of 39 hospitals with 3A Grade across the country,to find preferred regimen of the combination of Chinese and Western medicines.Results After screening,results suggested that:patients with no nervous system damage were recommended to use the combination of medicine:Xiyanping Injection with vitamins,non-steroidal anti-inflammatory drugs(NSAID),and external mucosal drugs;patients with nervous system damage:Xiyanping Injection with mannitol,Xingnaojing Injection,and glucocorticoids;patients with combination of pneumonia:Xiyanping Injection with antibiotics,heat-clearing antidote,and phlegm-resolving and antiasthmatic drugs.Conclusion A real world study of Xiyanping Injection combined with drugs in the treatment of herpangina and inpatients with complications were conducted.According to different disease stratification,therapeutic regimes using Xiyanping Injection combined with drugs are proposed,which are consistent with the clinical characteristics,disease changes and corresponding guidelines/consensus of the disease,and can be used as a reference for clinicians.
