基于区域护理专科联盟体的伤口造口居家服务模式的构建与实施

摘　　要：依托三级医院优质护理资源,构建基于区域护理专科联盟体的伤口造口居家服务模式,实现区域内同质化服务,提高居家护理服务质量。笔者所在医院护理部牵头吸纳8所基层医疗机构形成区域护理专科联盟体。主体医院对联盟医院伤口造口小组进行培训,符合资质并通过考核的护理人员在主体单位的指导下开展居家护理服务。2020年4月-2021年3月,主体医院伤口造口小组共开展了341例居家护理服务,其中伤口护理289例,造口护理52例。联盟医院伤口造口小组共开展了367例居家护理服务,其中伤口护理325例,造口护理42例。患者对居家护理服务的满意度为100%。可见,依托三级医院优质护理资源,构建区域内护理专科联盟体的伤口造口居家服务模式,实现了医疗机构资源共享,提高了基层护士居家护理服务能力,提升了护士自我价值能力,患者对居家护士的满意度高。Relying on the high-quality nursing resources of tertiary hospitals,the wound/stoma home service mode based on the regional nursing alliance was constructed to realize the regional homogeneous service and improve the quality of home nursing service.A regional alliance of nursing was set up by the nursing department of our hospital and other 8 hospitals,and home-based nursing training was conducted for nursing staff of the alliance.Between April 2020 and March 2021,the wound stoma team of our hospital carried out 341 home nursing services,including 289 cases of wound nursing and 52 cases of ostomy nursing.Meanwhile,the wound stomy teams of the alliance hospitals carried out 367 home nursing services,including 325 times of wound care,and 42 times of ostomy nursing.Patient satisfaction with home care service was 100%.It can be seen that,the nursing model studied in this paper can realizes resource sharing in medical institutions,improves the home-care ability and self-value of primary nurses,and enhances the patients′satisfaction.
