摘　　要：目的探讨MR形态学及扩散加权成像(DWI)特征在乳腺良恶性囊实性病变鉴别诊断中的应用价值。方法回顾性分析75个乳腺囊实性病变的形态学(肿块的形态、边缘)特征,并分别测得囊壁及囊腔中心区域的表观扩散系数(ADC)值进行统计学分析,绘制受试者工作特征(ROC)曲线,评估诊断效能。结果结合病理结果,75个病变中,良性32个,恶性43个。形态学诊断良性35个,恶性40个;DWI诊断良性33个,恶性42个;形态学联合DWI诊断良恶性囊实性病变的灵敏度、特异度、阳性预测值、阴性预测值及准确性均高于单一方法的诊断,差异有统计学意义。结论MR形态学、DWI特征在乳腺良恶性囊实性病变的鉴别中具有较高的灵敏度和特异度,二者联合分析可以进一步提高诊断准确性。Objective To investigate the value of MR morphological and diffusion weighted imaging(DWI)features in the differential diagnosis of benign and malignant cystic-vsolid lesions of breast.Methods Morphological characteristics(shape and edge)of 75 breast cystic-solid lesions were analyzed retrospectively.The apparent diffusion coefficient(ADC)values of cystic wall and central area of cystic cavity were measured and statistically analyzed.The differential diagnostic efficacy between benign and malignant breast lesions was evaluated by receiver operating characteristic(ROC)curve.Results According to the pathological results,there were 32 benign lesions and 43 malignant lesions among all of the 75 lesions.35 lesions were diagno.stic as benign and 40 lesions were diagnostic as malignant according to the morphological features.33 lesions were diagnostic as benign and 42 cases were diagnostic as malignant according to the DWI features.The sensitivity,specificity,positive predictive value,negative predictive value and accuracy of the morphological combined with DWI in the diagnosing benign and malignant cystic-solid lesions were higher than those of any single method with significant differences.Conclusion Both MR morphological and DWI characteristics have high sensitivity and specificity in the differential diagnosis of benign and malignant cystic-solid lesions of breast.The combination of the two methods can further improve the diagnostic accuracy.
