强直性脊柱炎患者MRI表现与BASDAI评分的相关性

摘　　要：目的探讨强直性脊柱炎(AS)患者MRI表现及Bath强直性脊柱炎病情活动指数(BASDAI)评分的相关性。方法选择121例AS患者纳入研究,所有患者均行MRI、扩散加权成像(DWI)及动态对比增强磁共振成像(DCE-MRI)扫描,进行MRI评分[加拿大脊柱骨关节研究协会(SPARCC)评分],测定表观扩散系数(ADC)、对比剂自血管至组织间隙渗透率(K^(trans))、细胞外间隙容积与血管内容积比(K_(ep))、细胞外间隙容积(V_(e))等指标,进行BASDAI评分。结果活动期患者SPARCC评分及ADC、K^(trans)、K_(ep)、V_(e)等指标值显著高于稳定期患者(P<0.05);Pearson相关性分析显示,骶髂关节SPARCC评分及ADC、Ktrans、Kep、Ve等指标与BASDAI评分呈正相关(P<0.05)。结论MRI参数与BASDAI评分呈正相关,SPARCC评分、DWI参数及DCE-MRI参数可作为评估病情的有力指标。Objective To investigate the correlation between MRI findings and Bath ankylosing spondylitis disease activity index(BASDAI)score of patients with ankylosing spondylitis(AS).Methods A total of 121 patients with AS were enrolled in this study and subjected to MRI,diffusion weighted imaging(DWI)and dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI).The Spondyloarthritis Research Consortium of Canada(SPARCC)scores were recorded,apparent diffusion coefficient(ADC),contrast transfer coefficient(K^(trans)),reflux constant(K_(ep)),extravascular extracellular volume fraction(V_(e))were measured,and BASDAI was used for scoring.Results Patients in active stage had significantly higher SPARCC scores,ADC,K^(trans),K_(ep),and V_(e)than those in stable stage(P<0.05).Pearson correlation analysis showed that the above indexes were positively correlated with BASDAI score(P<0.05).Conclusion There is a positive correlation between MRI parameter and BASDAI score.All of SPARCC score,DWI parameter and DCE-MRI parameter can be used for disease evaluation.
