经脐单孔腹腔镜巨大子宫切除术34例临床分析

摘　　要：目的:探讨经脐单孔腹腔镜在巨大子宫(16~20孕周大小)切除术中应用的安全性可行性。方法:本次分析选取2019年1月~2021年6月在我院完成的经脐单孔腹腔镜下大子宫除术共34例进行分析。患者平均年龄(42.3±2.5),其中子宫腺肌症14例,子宫平滑肌瘤20例。结果:所有34例患者均完成手术,成功率100%。平均子宫重量(920±162)g、手术时间(115±38.3)min、出血量(73.2±34.6)mL,术后平均肛门排气时间(20.7±11.2)小时,术后24小时疼痛VAS评分(3.2±1.2)分,术后平均住院时间(4.9±1.2)天。结论:在具备熟练的单孔腹腔镜手术经验基础上,经脐单孔腹腔镜应用于16~20孕周大小子宫切除术安全可行。Objective To explore the safety and feasibility of transumbilical single-port laparoscopic hysterectomy for giant uterus (16~20 gestational weeks).Method This analysis selected 34 cases of transumbilical single-port laparoscopic hysterectomy in our hospital from January 2019 to June 2021.The average age of the patients was (42.3±2.5),including 14 cases of adenomyosis and 20 cases of uterine leiomyoma.Results All 34 patients completed the operation with a success rate of 100%.The mean uterine weight was (920±162) g,the operation time was (115±38.3) min,the blood loss was (73.2±34.6) ml,the mean anal exhaust time was (20.7±11.2) hours after operation,the pain VAS score was (3.2±1.2) in 24 hours after operation,and the mean postoperative hospital stay was (3.2±1.2).Conclusion On the basis of skilled experience in single-port laparoscopic surgery,transumbilical single-port laparoscopic hysterectomy for 16~20 gestational weeks is safe and feasible.
