摘  要:目的:了解长沙市阿奇霉素耐药淋球菌NG-MAST分型及相关基因多态性。方法:收集2018~2020年长沙市淋球菌分离株,用琼脂稀释法选出阿奇霉素耐药菌株(AZ-R,MIC≥1 mg/L),并随机抽取相应数量敏感株(AZ-S,MIC≤0.25 mg/L)作为对照,同时进行NG-MAST分型及23S rRNA、erm、mtrR基因突变位点分析。结果:共收集到155株淋球菌,其中耐药株23 株,耐药率为 14.8%;NG-MAST共获得47个ST型,41个由单一菌株构成,含27个新型别;2株耐药株23S rRNA发生错位突变,分别为A2059G和C2611T在4个等位基因全突变;未发现带有erm基因的耐药株;mtrR基因启动子反向重复序列中单个核苷酸(A)缺失AZ-R组占比高于AZ-S组。结论:目前,长沙市单独采用阿奇霉素治疗淋病已不适用;mtrR基因核苷酸(A)缺失与阿奇霉素淋球菌耐药存在显著相关性,有助于我们对其流行特征进行分析和监测。Objective To investigate the NG-MAST genotype and related gene polymorphisms of Azithromycin resistant gonorrhoeae in Changsha.Methods Clinical isolates of gonorrhoeae in Changsha from 2008 to 2020 were collected.Azithromycin resistant strains (AZ-R,MIC≥1 mg/L) were selected by AGAR dilution method and the same number of sensitive strains (AZ-S,MIC≤0.25 mg/L) were randomly selected as the control group.NG-MAST typing and 23S rRNA,erm and mtrR genes mutation sites were analyzed .Results In total,155 strains were collected and 23(14.8%) strains were resistant to Azithromycin resistant.There were 47 ST types and 41 of them were composed of single strain.27 new ST types were found.The 23S rRNA of two drug-resistant strains was misaligned with A2059G and C2611T in four alleles,respectively.The proportion of single nucleotide (A) deletion in reverse repeat sequence of mtrR gene promoter in AZ-R group was significantly higher than that in AZ-S group.No resistant strains carried erm gene.Conclusion At present,azithromycin is no longer suitable for the treatment of gonorrhea in Changsha.The deletion of mtrR gene nucleotide (A) is associated with azithromycin gonorrhea resistance.This will help us to monitor the epidemiology of Azithromycin resistant gonorrhoeae.
