摘 要:目的:探讨术前评估磁共振神经成像(MRN)对经皮椎间孔镜手术(PETD)治疗腰椎间盘突出症的效果评价。方法:收集2016年1月~2020年12月期间接受PETD治疗的50例腰椎间盘突出症患者,根据术前是否使用MRN评估分为MRN组(25例)和非MRN组(25例)。对比分析两组患者的疼痛数字评分(NRS)、Oswestry功能障碍指数(ODI),并发症和复发率。结果:MRN组和非MRN组患者术后1周、术后1月、术后3月、术后6月NRS和ODI评分均较术前明显降低。MRN组术后3月和术后6月的NRS和ODI优于非MRN组。MRN组并发症优于非MRN组。结论:MRN技术可对经椎间孔镜下腰椎间盘髓核摘除的安全工作区域进行术前评估和判断责任病灶,对减少神经损伤并发症具有指导意义。Objective To evaluate the value of magnetic resonance neurography (MRN) in the preoperative evaluation of patients with lumbar disc herniation treated by percutaneous endoscopic transforaminal discectomy (PETD).Methods A collection of 50 patients with lumbar disc herniation who received PETD treatment from January 2016 to December 2020 were divided into MRN group (25 cases) and non-MRN group (25 cases) according to whether MRN was used for preoperative evaluation.The numerical rating scale (NRS),Oswestry disability index (ODI),complications and recurrence rates of the two groups were compared and analyzed.Results The NRS and ODI scores of patients in the MRN group and non-MRN group were significantly lower at 1 week,1 month after PETD,3 months after surgery,and 6 months after surgery.The NRS and ODI of the MRN group at 3 months and 6 months after the operation were better than those of the non-MRN group.Complications in the MRN group were better than those in the non-MRN group.Conclusion The MRN technology can perform preoperative assessment and judge the responsible lesions for the safe working area of lumbar disc nucleus pulposus excision under PETD,which has guiding significance for reducing nerve injury complications.
