益肾柔肝方联合屈螺酮炔雌醇治疗体质指数正常多囊卵巢综合征疗效及对激素水平、卵巢功能的影响

摘　　要：目的观察益肾柔肝方联合屈螺酮炔雌醇治疗体质指数正常多囊卵巢综合征患者疗效及对激素水平、卵巢功能的影响。方法将2019年6月—2021年1月在石家庄市中医院妇科治疗的90例正常体质指数多囊卵巢综合征患者随机分为西药组30例、中药组30例、中西医结合组30例。西药组给予屈螺酮炔雌醇治疗,中药组给予益肾柔肝方治疗,中西医结合组给予屈螺酮炔雌醇和益肾柔肝方治疗,3组疗程均为12周。检测3组患者治疗前后血清抗苗勒管激素(AMH)、促卵泡生成素(FSH)、促黄体生成素(LH)、睾酮(T)水平和卵泡数量、双侧卵巢体积,评价3组患者治疗前后中医症状积分,比较3组患者临床治疗总有效率,记录3组患者随访6个月内排卵率、妊娠率。结果治疗后,3组患者血清AMH、LH、T水平及窦卵泡数量、两侧卵巢体积、中医症状积分均明显低于治疗前(P均<0.05),中西医结合组上述指标均低于其他2组(P均<0.05),中药组均低于西药组(P<0.05);3组患者血清FSH水平高于治疗前,中西医结合组高于其他2组(P均<0.05),中药组高于西药组(P<0.05)。西药组、中药组、中西医结合组总有效率分别为60.0%(18/30)、76.7%(23/30)、93.3%(28/30),中西医结合组治疗总有效率显著高于其他2组(P均<0.05),中药组高于西药组(P<0.05)。随访西药组排卵率和妊娠率分别为50.0%(15/30)、36.7%(11/30),中药组分别为63.3%(19/30)、50.0%(15/30),中西医结合组分别为80.0%(24/30)、66.7%(20/30),中西医结合组排卵率和妊娠率显著高于其他2组(P均<0.05),中药组高于西药组(P均<0.05)。结论益肾柔肝方联合屈螺酮炔雌醇治疗正常体质指数多囊卵巢综合征患者的临床疗效显著优于单纯西药和中药治疗,可明显改善症状,提高排卵率和妊娠率,机制可能与调节激素水平有关。Objective It is to observe the efficacy of Yishen Rougan decoction combined with drospirenone and ethinyl estradiol in the treatment of polycystic ovary syndrome patients with normal body mass index and its effect on hormone levels and ovarian function.Methods A total of 90 patients with normal body mass index polycystic ovary syndrome who were treated in the gynecology department of Shijiazhuang Hospital of Traditional Chinese Medicine from June 2019 to January 2021 were randomly divided into western medicine group,traditional Chinese medicine group,and integrated traditional Chinese and western medicine group,30 cases in each group.The western medicine group was treated with drospirenone and ethinyl estradiol,the traditional Chinese medicine group was treated with Yishen Rougan decoction,and the integrated traditional Chinese and western medicine group was treated with drospirenone and ethinyl estradiol and Yishen Rougan decoction,all the groups were treated for 12 weeks.The levels of serum levels of anti-Müllerian hormone(AMH),follicle-stimulating hormone(FSH),luteinizing hormone(LH),testosterone(T),the number of follicles,and bilateral ovarian volume of the patients were detected before and after treatment in the three groups,and TCM symptom scores of patients before and after treatment were evaluated in the three groups,the total effective rate of clinical treatment was compared among the three groups,and the ovulation rate and pregnancy rate within 6 months of follow-up in the three groups were recorded.Results After treatment,the levels of serum AMH,LH,T,number of antral follicles,bilateral ovarian volume,and TCM symptom scores were significantly lower than those before treatment in the three groups(all P<0.05),the above indexes in the integrated traditional Chinese and western medicine group were lower than those in the other two groups(all P<0.05),and the traditional Chinese medicine group was lower than the western medicine group(P<0.05);the serum FSH level of patients was higher than that before treat
