产前应用不同剂量硫酸镁对早产儿神经保护的临床观察

摘　　要：目的比较孕妇产前应用不同剂量硫酸镁对早产儿神经保护的临床疗效。方法选择我院收治的接受硫酸镁治疗的早产儿86例,按照孕母产前硫酸镁使用剂量分为两组,对照组40例使用剂量<15 g,观察组46例使用剂量≥15 g。比较分析两组早产儿出生3天血清镁离子浓度、出生后7天振幅整合脑电图、纠正胎龄36周时头颅核磁共振、纠正胎龄40周时新生儿行为神经评分(NBNA)。结果两组血清镁离子浓度、振幅整合脑电图、头颅核磁共振及NBNA评分比较,P<0.05。结论产前应用硫酸镁累计剂量≥15g对早产儿神经保护疗效显著。Objective To compare the clinical effect of different dosage of magnesium sulfate before delivery on neuroprotection of preterm infants.Methods 86 preterm infants treated by different dosage of magnesium sulfate in our hospital were selected and divided into two groups according to the dosage,40 cases in the control group were with the dosage<15 g and 46 cases in the observation group were with the dosage≥15 g.The results of serum magnesium concentration of preterm infants at the 3 rd day of birth,amplitude integrated EEG at 7 th day of birth,head MRI at 36 weeks of gestational age,and neonatal behavioral neural score(NBNA)at 40 weeks of gestational age of the two groups were analyzed and compared.Results There were statistically difference in serum magnesium concentration,amplitude integrated EEG,head MRI and NBNA score between the two groups(P<0.05).Conclusion The dosage of magnesium sulfate≥15 g before delivery has significant neuroprotective effect on preterm infants.
