血液超滤辅助治疗HFrEF袢利尿剂抵抗患者的疗效

摘　　要：目的探究血液超滤辅助治疗射血分数降低型心力衰竭(HFrEF)袢利尿剂抵抗患者的效果。方法将117例HFrEF袢利尿剂抵抗患者采用抽签法分为两组,对照组58例给予阶梯式利尿剂药物治疗,观察组59例在对照组基础上给予血液超滤治疗,对比两组患者心功能、液体潴留情况、电解质水平。结果治疗前两组左心室收缩末期内径、左心室舒张末期内径、射血分数及氨基末端脑钠肽前体(NT-proBNP)水平比较,P>0.05;治疗后观察组的左心室收缩末期内径、左心室舒张末期内径、NT-pro-BNP水平低于对照组,射血分数、体液丢失、体重减轻高于对照组(P<0.05)。两组治疗前后电解质水平比较,P>0.05。结论血液超滤辅助治疗对HFrEF袢利尿剂抵抗患者,可以通过对患者液体潴留情况的改善,促进心功能的恢复,同时对电解质指标无显著影响,具有较高的安全性。Objective To explore the effect of adjuvant treatment of blood ultrafiltration in patients with heart failure with reduced ejection fraction(HFrEF)and diuretic resistance.Methods 117 patients with HFrEF and diuretic resistance were selected and randomly divided into two groups.58 cases in the control group were treated with step diuretic drug therapy,and 59 patients in the observation group were treated with blood ultrafiltration on the basis of the control group.The cardiac function,fluid retention,and electrolyte levels of the two groups were compared.Results Before treatment,there was no significant difference in left ventricular end-systolic diameter,left ventricular enddiastolic diameter,ejection fraction,and N-terminal brain natriuretic peptide(NT-pro-BNP)levels between the two groups(P>0.05).After treatment,The left ventricular end-systolic diameter,left ventricular end-diastolic diameter,and NT-pro-BNP levels of the observation group were lower than those of the control group;ejection fraction,fluid loss,and weight loss were higher than those of the control group(P<0.05).There was no statistically significant difference in electrolyte levels between the two groups before and after treatment(P>0.05).Conclusion Adjuvant treatment of blood ultrafiltration can improve the patient’s fluid retention and promote the recovery of heart function for patients with HFrEF and diuretic resistance.At the same time,it has no significant impact on electrolyte indicators with high safety.
